Very simple. Your first opportunity is at the very beginning when your given your first dialogue choice. Instead of choosing one of the options hit the left bumper and you'll drawer your sword on the poor fools trying to help you. Then quickly chase one down and run him through.
If you choose a dialogue option that makes you pass out he out side when you wake up and either run through Munechika or Osei, or run out the gate and pick a fight with the would be rapist just outside. Either way once the first person falls to your blade this achievement pops.

Grummy
As basic as it gets. This is impossible to miss and you'll likely get this within minutes of starting. It must be a kill, not a blunt attack. As with the Blunt Attack achievement, Villagers don't always count, so kill someone who has a weapon.
