For this or any other endings/difficulties there is a trick to becoming much more powerful you can go to the accesory store (its in the point next to the village connecting it to the gate sorry its name eludes me) and buy a health permanent increase item (Its a green necklace) and unlike the previous Way of the Samurai installments your health will always stay increased. You can buy as many as you can (9), use them, buy more and repeat till your out of money then Mug/Kill the shopkeeper leave Amana and repeat until your health maxes at 9999

Clearing the game requires a single play through with an ending. Death doesn’t count. These achievements do not stack, so you will have to get each one individually. Hard difficulty isn’t especially hard if you’re prepared, however some people do find it to be very irritating and frustrating. In that situation, wait until you’ve unlocked all the endings, then pick one you found to be easy or quick and use that one. Some endings, such as ending 20, require very little combat, and that can simplify this achievement immensely.
