To unlock the achievement, you must maintain
untill the guitar release a yellow wave. The wave will knock off the ennemies near you, attack them by pressing multiple times while they are on the ground.

Does this need to be 15 different NPCs? Do you have to kill them while they're on the ground or just hit them once with a regular attack?

The description for this appears to be wrong. You can find Youtube videos of people getting this achievement very early in the game, before even unlocking the Charged Facemelt ability...and indeed, following those is what finally unlocked this for me. I attacked umpteen people with charge-followed-by-RT-spam and no achievement. It appears what the game is actually looking for you to do is stun someone with Vibe (LT) and then blast them with regular attacks (RT) while they are stunned. Killed 15 people with this method and the achievement popped.

The ability to do a charged facemelt unlocks as part of the story.
Once you unlock it hold R2 until a yellow wave is released knocking down the hostile NPCs in front of you. When they are on the ground press R2 rapidly to defeat them.
After 15 are defeated the trophy will unlock.
