During the Pigman chapter your main goal is to burn three roots of the Soul Tree. After you burn the third root, Ruth appears and will attack you (with two other Pigmen).
After you defeat her, the Hethen will appear and give you the option to turn Ruth into a Pigman. Choose yes and the achievements will pop.

The heathen isn't appearing for me after I kill ruth.
