This achievement unlocked for me on level 5 - Full Steam Ahead.
I used the second weapon that you'll unlock that fires a spiked ball. Just wait until you find 4 enemies together and you will easily unlock this.

This "solution" is pretty awful. I tried using the spike ball on 4 enemies in several different places, and never got the achievement to pop. Noticing that the name of the achievement was "Unerring Explosion" I switched to the sticky grenade gun and got it first try. There are several spots in level 4 and 5 where you can find a group of 4 or 5 enemies. I got mine to pop by killing 4 of the enemies that look like cops. Not sure if you can do it on the spiders, or the flying drone things. Hope this helps anyone who is having trouble.

Sorry the guide wasn't of any use to you. I vaguely remember popping this one by pure chance while playing through.
