How to unlock the Extra ball! achievement in Werewolf Pinball - Definitive Guide

On this table, the Extra Ball can be earned by hitting the house on the left side of the board, just above and to the left of the left flipper bumper, six times, and then completing a timed shot mode that requires six lanes to be passed through.

This can be done across multiple balls but must be completed in one game, and losing a ball after hitting the house 6 times will cause you to restart the process.

Hitting the house is a tricky shot, probably best done using the right flipper right off the end. It can also be hit with some luck by the left flipper INTO the right flipper bumper, but it's not exactly easy to make happen. I've also had it bounce into the house off the right flipper bumper when the ball was dropping down with some speed, pure luck.

Once you've hit the house 6 times (all six blue bubble lights in front lit up), it will spawn the ball over your right flipper. You'll then be in the timed mode, you have six lane shots to make, with about 30seconds to make each. I'm pretty sure the lane order stays the same each time, though I did once or twice notice it seemed different; however, I think I simply made a second lane shot immediately after the other and it ticked over to a third and just appeared like the lane never lit (and I did not complete the mode on any of these attempts).
It starts with the longest lane on the left, then over to the lane just to the right of the bumpers up top; after that, it lights up above the WEREWOLF lane opening, which I found tricky to get to 'count'. Next it goes over to the right hand lane, then finally the right flipper lane, and ends at the left flipper lane. You can see which lane is targeted by looking for the orange blinking light under or over the lane in question.

Upon hitting the 6th target lane, your Extra Ball unlocks and so does your achievement.

Using the very end of the bottom right flipper was by far the easiest method for me. I would just hold the RT to trap the ball, wait till it came to a rest and then perform the shot.

Plus, when you successfully make it, the ball is returned on the right side, so I was able to get into consistent rhythm and put in 4-6 balls in a row once I got the feel for it.
