How to unlock the Pacifist achievement in Westerado: Double Barreled - Definitive Guide
I, unfortunately, could not get this in the manner that Calex dEUS did so I am providing a quick no kill (up until the murderer's hideout) walkthrough. After finishing with the murderer my stats show about 15 minutes of game time. I've gone ahead and added achievements you will unlock if this is your first time doing any of these story lines. It helps to have played some and are familiar with the map.
Start as the main character and choose to skip the tutorial, if you can. Head to Clintville and help the lady with the drunk husband, and then see the Sheriff for his clue.
Go talk to the Old Miner and ask about his hat. Go to the Train Station and talk to the ticket salesman and ask about a specific ticket. Now talk to the Station Master and get the mission to clear the tracks. Return to the Old Miner and get his hat.
Head to the Ghost Miner and walk with him to the Beefhead gang to clear the tracks (the ghost miner will scare them away). Now take him up to his pick axe to get his clue.
Go back to the Train Station and talk to the Station Master and she will give another clue. Now you need to find three hands to help clear the tracks of debris. Three guaranteed helpers are Rancher Richard (Southeast of Clintville) and Rancher Bell and the Old Miner (both near each other in Clintville). Go talk to each one and then go straight into the tunnels to meet the Station Master and the three hands. Return to Clintville Station and talk to the Station Master once more.
Talk to the Ticket Salesman and ask about a timetable. Tell him you are from Transcontinental Express.
Talk to the Station Master again and she will chew out the ticket salesman. Talk to her one more time and she will give you another clue and tell you that your sister is headed to town. You will then have to wait for the Train to show up with your sister.
The quickest way to make the train show up is to just head west on the tracks until you get to Santa Anna Station. Now head North to the Haberdasher and ask her about the business and then offer to find her hands to run the store. Now go out and talk to all of the women in Santa Anna and Santa Anna Station until you find three that you can ask to help, then return to the Haberdasher.
Fast travel back to Clintville Station and talk to your sister.
You now have to meet her at the following places in order, where she will give you a clue that she learned:
1. Your Ranch (Where the game starts)
2. Fort Motors (East of Clintville Station)
3. The Cemetary (North of Clintville)
4. The Desert (One Screen East of Santa Anna Station)
5. Lake Santa Anna (Cross the Bridge to the West from Santa Anna, go North two screens and then cross a bridge back East)
6. Finally, find her at the bridge back in to Santa Anna.
For me the murderer showed up almost immediately upon fast-traveling back to Clintville. Now confront him and get to the Murderers Hideout.
Upon arriving and during the first dialogue with the murderer, get a good look at him to verify his appearance in case you didn't get every clue (I didn't have his bandana color).
Now there are 27 people to deal with including the murderer in his hideout. There are 6 in the first section where you flip the two switches. These can easily be avoided by sprinting through the section.
After you come back out of these mines, you have 21 people left that have to die. This is not a problem as it's almost guaranteed that two will be killed in crossfire. But I just ran around a bit for the first five until at least two were dead, and then killed the other three.
Now it is very important to press for the map and then to the image of the killer to refresh your memory. A few differences that seem to be most prevalent are his hat style, his bandana color, or lack thereof, and the belt and buckle colors. Not as common were fat or thin and shirt color. Now you have to fight four guys in this room and you should be able to get at least one or two crossfires here too.
Next is the horse chase. You have 11 guys here and just from avoiding them and reloading you should pick up a couple more crossfires.
Finally complete the boss fight where you can chose not to kill the murderer and unlock the achievement along with these:
Note: You can die during any point in the Murderer's Hideout and your kills while in the hideout will reset when you start again.

I got this today at the end of my first play through , and it's either bugged in a good way or there is a second way to unlock it other than killing less than 20 people. It popped after 2 other achievements right at the end and I had the 250 kills achievement in this run so clearly I killed more than 20 people.

Maybe thats what unlocked this , if any one on a high kills run can verify this it would be great.
UPDATE: UnsaferLlama7 has confirmed he got this the same way, die in the final section reload the save and it should be yours. Seems to reset the kills count on this achievement when you reload the game.

There's 27 people in the murderer's hideout so I'm assuming you need to ignore the guys in the first part with the switches and let two or three others shoot each other in a crossfire.
