How to unlock the Surname Frank achievement in Westerado: Double Barreled - Definitive Guide
WARNING
The Indian quest line includes the following missions.
1. Talk to the Indian Chief, then release three buffalo herds.
2. Return to the Chief and then take the token of trust to the Chief's Brother.
3. Fight the Military at the North side of the Fort.
4. Fight the citizens of Clintville in Clintville.
5. Get enough of the clues that don't trigger any other endings.
6. Identify the murderer and complete the ending battle at his hideout.
If all else fails RedmptionDenied mentioned this in the comments: "Since the dev said the ending itself was randomly chosen I tried just reloading the final checkpoint after not getting it, repeating the boss fight, and sure enough it triggered this one with the exact same missions done."
"Surname Frank" may or may not unlock after doing the Indian quest line, but it is the only way to unlock it. A developer mentioned in a thread on Steam that some of the quests for the Indians overlap with "Shoulda Took Off," therefore there is a fifty percent chance that you will get either ending. In the same thread the developer recommended the following steps, in parenthesis, if you keep getting the "Shoulda Took Off" ending. (Talk to the Chief and then free the Buffalo, then return to him and take the token to the Chief's Brother, NTS to Fort Motors and kill everyone in the Fort including Lt. Fort and Gen. Motors by yourself, and then go directly after the Murderer.)
This achievement results from completing the Indian quest line. My advice to avoid getting the wrong ending is to do this only with the Hero, which is your starting character. If you still have trouble, try the steps in the Warning above.The Indian quest line includes the following missions.

FINALLY. Rant incoming. Had to do 2 playthroughs specifically for this achievement and maybe I'm just super unlucky, but this took me maybe 12 total boss sequence finishes.
1st playthrough I may have done other quests that could've somehow negated this achievement but I don't think that's the case. I was able to make it to the end and killed the murderer but he mentions something about being "hired" and "buffalos"... click, click, click. Retried the boss fight sequence ~8 more times. No conversation at the end after beating the boss. Straight to credits and doesn't give me the option to run up and click "A".
2nd playthrough I stuck to the above-mentioned quests and route. However, the issue this time was being unable to get The Murderer to spawn. It appears I was a couple of quests short of him spawning because I got the clue from The Director and the Golden Buffalo and found him almost immediately. The next issue was like the 1st playthrough when defeating the boss, he mentions the "hired/buffalo" nonsense. Retried led to NO conversation afterward and straight to credit. The 3rd time was the charm and proc'd the "halfblood" gibberish for this achievement.
TL;DR - Minimal quests required for the murderer to spawn. Stick to the list above. Retry as many times as you're willing or you may have to do a new playthrough based on luck...

Sigh... Played through normally, and I definitely got the right ending. All the text made sense, but the achievement still didn't pop...
Interestingly, all 3 times I tried following the steps in parentheses, I got the "shoulda took off" ending, not this one...
