Chapter 4: at the end of the chapter, after talking to Leo's crew in the stage during the play, Everett and the boy will evade through the sewers. You'll need to to flee Nash and at the end of the run you'll find Everett at the other side of a gap. After a short dialogue, you'll jump over and during the slow-mo a prompt will appear on the screen asking you to press a specific button. In my case, it was . Simply press it during the timeframe.

If both QTE are done, achievement should unlock right after the second one.
