From what I can tell this is pretty random and you do not really need to do anything to get it to appear. While playing a level you may see a red arrow pointing you off screen like the ones that appear pointing you to chests. Follow them and you will see a sword. Just grab it and the achievement will unlock.
Once you have the sword it will appear above your XP bar in the bottom right corner of the screen. What the sword does is when you die on the run you pick it up on it will immediately give you half health back. You don't get a death screen or anything you will just get the health so make sure you notice when it's used.
