A toss is performed by pressing just before catching the disc. If you have the good timing, the disc will be sent in the air.

When we press , an animation is triggered, you want to receive the disc during this animation. If the timing is perfect, this is a perfect toss and the disc goes directly above you, otherwise it will fly by to another direction.

If you have 2 controllers, you can practice by playing against another player. Send the disc and wait for the automatic return.

Sliding is done by simply pointing the left stick in any direction and pressing . You must do it without the disc in your possession. It is very useful for defending opposing shots. All you gotta do for the cheevo to pop is use the sliding mechanic 151 different times (it's cumulative across games)

P.S. the achievement is a reference to the Pokémon franchise. Originally there were 151 Pokémon which was accompanied by the infamous phrase "Gotta Catch 'Em All!" - hence the cheevo name, "Gotta Toss Them All!"
