Ending D (a.k.a. the "True Ending") is the hardest ending to achieve, and can be gotten only in New Game +.
Notice the eyes coming from the West Basement Wall when you start a new run; talk to them, and fulfill that linear quest to get Ending D.
Note that you should be powerful enough to fight harder enemies!
