This requires you to take a random cursed artefact in 4 separate stages.
Cursed artefacts give you some form of bonus but at a cost.
HOWEVER, this is entirely RNG. I've made it to 3-3 multiple times now and never once gotten it.
The reason for this is that there are three "stores" per non-boss stage but those are random from a list of 8.
1 - Relics and potion
2 - Arcana
3 - Pinata
4 - Card swap
5 - Relic buyer
6 - Med bay
7 - Clothing merchant
8 - Cursed Relics
Each floor will have a Green, Purple, and Red shop, denoted by the color of the portal that appears in the shop. The green will always be Artisan Andres, and the purple will always be Virtuoso Iris, but the red portal will be a random one of the other NPCs.
So per run there are only 6 chances to get the cursed relic shop, you only have a 1/6 chance to get it each time, AND you have to get it on 4 separate stages as you are only allowed to take 1 of the 3 cursed relics each time.
Below is a list of all 27 Cursed Relics and their effects.

UPDATE [21/8/19]:
There have been a number of patches since submitting this solution that have made it much easier to get this achievement.
- In the area where you start your run, there is now a cursed Relic shop in the top-left [north of the relic collection chest, left of the dungeon portal.] In exchange for crystals you can buy 1 of 3 relics to start your run with and increase your chances of getting all 4 that you need.
- New Relics were added to the collection pool, including NPC tokens that will make the specified NPC spawn as one of the three in the dungeon. This includes one for the cursed relic seller, so entering the dungeon with this and starting with a bought cursed relic will get you half-way there at the start. [NOTE: I've not actually used these tokens myself so do not know if they are single use, automatic, or if they have to be dropped to spawn the NPC.]
- They also added at least 1 more NPC, maybe more, a guy who will give you all his crystals for all your gold. So now there's an even lower chance of getting the cursed relic NPC to spawn per non-boss dungeon floor.

Just got it yesterday, theres a chance the seller can sell a curse item that makes all non curse relic into a curse relic. i had 9 at the time and achievement pop as soon as i got curse relic.

Update: There is a small chance that a cursed relic will drop.

Cursed relics can be obtained from Nox the Unfortunate who gives one relic for free or by defeating enemies inside the Chaos Trials. Note that enemies and bosses drop the relics very rarely, thus your best bet is to find Nox inside the Trials. These relics are extremely powerful but they debuff the player as well. I suggest doing this at the same time as Gotta Go Fast. Get your hands on the Pathfinder's Map relic and get inside the Trials. Press 
to view the map and see if the NPC is there. If he is on the map, get to him and take a cursed relic with you. If he is not on the map, then go straight for the mini-boss and so on. He looks like this (credit to the official wiki):
