You will receive a sworn brothers boon or set of armor after you reach an oath level of 10 with that companion. You raise a companions oath level by killing enemies together.
Easiest way to raise a companions oath level is by going back and completing main battlefields or by completing sub battlefields with that companion, or any time you do not have two companions during the main questline. You will get a notification once you become sworn brothers with a companion.
After becoming sworn brothers at any battle flag go to down to deliveries and you will notice you have a new delivery from that companion and once you click on it the achievement will unlock.
EDIT: You can also you the item "Cup of Cordiality" in the reinforcements tab at any battle flag to raise companions level.

You should add that you can use cups of cordiality to increase your oath level as well. You get a full level with one cup.
