The maps are indeed handy, but if you're new to this game it can take some getting used to, so here are some general tips:
- The best way to play this game is not to run and gun in the way you would in Doom, since a few good hits will be enough to kill you on the higher difficulties. To succeed you need to be systematic and cautious, clearing one room at a time. Do not try and circle strafe or run past enemies.
- By far the best tactic is to hide around a door or corner and pick of enemies one by one as they try to follow you. If this isn't possible try to fight at medium distance, since enemies are fairly inaccurate from there.
- Fighting a group of enemies head on isn't a good idea since their bullets pass through eachother and there is no form of infighting, which was always so vital to surviving Doom's higher difficulties. Both your shots and their will pass through pillars and any other scenery.
- Once "woken" enemies will head for your position and never make another sound (the green suited mutants make no noise at all) so it is surprisingly easy to be outflanked.
- Save often. Make one file for the beginning of the level then systematically use another one as you progress. A few times the game crashed for me on the level summary screen, so may be worth saving just before the exit.
- Assuming you don't care about a high score, large portions of certain levels can simply be skipped. Focus on getting the key(s) then reaching the exit, not killing every enemy and finding ever secret.
- The machine gun should be your default weapon for 90% of the game, with the chaingun saved for special occasions or bosses. The knife and pistol are utterly useless for anything aside from dogs so always retreat and find more ammo rather than try and fight with them. That said, ammunition is fairly plentiful as long as you don't adopt the spray n'pray tactic.
- Use the same tactics for all bosses: get a few good hits in, hide around the corner whilst they fire, then pop out for a few more shots with the chaingun. If there are any other enemies present then deal with them first, and ensure you activate any secret areas first to avoid running low on health and ammo
- The game is rather fussy about positioning for item pickups, so try and walk forwards over them wherever possible

I'm doing this without saving, at least without saving past the beginning of each level. I know, I'm a masochist. Let's just say I already made it to the fifth episode without save-scumming thus far.

Altough Wicelow's very nice solution for this achievement, you only need to complete 54 of the 60 levels.
The other six are bonus-levels and don't count towards this achievement, and are totally optional.

Not true, i completed all episodes.. got all achievements.. but not this one. I am missing the bonus floor on episode 3/4/6 according to the leaderboards so im guessing thats the bug?

It is true. Secret levels are not required. To Jerick85, play through each level individually again on Bring 'em On until you hit the jackpot is all the advice I can give you.

Difficulty do stacks so start on this difficulty from the beginning to save you some time.
Check every secret room for additional health/guns/ammo.
