How to unlock the Escaped from Wolfenstein achievement in Wolfenstein 3D - Definitive Guide
Complete all floors in episode 1.
Only "Don't hurt me" difficulty is required but to gain as well the "Blazkowicz-ed!" achievement at the end, start directly with the "Bring 'Em On!" difficulty.

And don't let Secret Floor 10 fool you into thinking you can skip Floors 2 thru 9. You still have to do them all after exiting Floor 10.

Got this yesterday but took me longer than I thought. Back in the 90's I just rushed through the floors I think :D

Complete all 9 floors of Episode 1.
It requires that you play on at least "Don't Hurt Me" difficulty, but its best to play on "Bring 'Em On!" difficulty so you can also net this achievement after finishing all episodes:

These wikia maps are much more useful than the gamefaqs maps, as they show all item/ key/ secrets/ etc.
Here is a brief legend for the more important info:
-Gold "k", Teal "k" are keys (doors will be colored if they require a certain key)
-Teal "g" is a gun, whether it be Gatling or Machine.
-Green square and arrow is starting point
-Gold squares are the exit
-Squares outlined in white are secret doors
-Red "+" are med kits
-Small teal squares are ammo
-Small teal circles are health plates (+10 health)

Simply complete the first episode to gain this achievement.

Complete episode 1, Escape from Castle Wolfenstein. I’d recommend completing this on Bring ‘Em On instead of Don’t Hurt Me so you can get the Blazkowicz-ed trophy as well and not have to play through the episode twice. For a look at the boss:
