The Tough Skin perk reduces BJ’s health loss from melee attacks.
In order to unlock this perk, you must perform the following requirement at anytime during the Campaign:
- Obtain 10 pipe head explosions
TIP: You will not be able to start working on this perk until Chapter 6, Ruins. Once you're on this Chapter, simply use your pipe (attached or detached) to melee the zombie enemies. If you aim for their head during the melee swing, their head will explode and it will count towards the requirement. There's more than enough zombies from Chapter 6 onward, so this will come naturally so long as you use your pipe when combatting them.

Requirement: Obtain 10 pipe head explosions
In Chapter 6, you will start to face a new type of enemy. You can only obtain a pipe head explosion on these types of enemies. Simply equip your pipe weapon in two hands, i.e. have it screwed together as opposed to two separate pieces. All you need to do is aim your melee attack at the new enemies heads and if done correctly their head will explode on impact. Once you have killed 10 enemies this way the perk and the trophy will unlock.
To track your progress towards this trophy, press the pad and navigate to the ‘Perks’ tabs.

It is best to get this trophy at the first meeting with a zombie Nazi (Chapter 6 “Ruins” at the very beginning), there will be a checkpoint in front of him and you can safely reboot until the final result: 10 broken heads (the head should burst) 
PS You need to do it with a full pipe and it’s better to hold it in your hand as your main weapon! You should hit it squarely in the face, but if you hit the helmet, it will protect the zombie and the head will not explode
