When you first get control of your pistol, coming off the elevator, take out the 2 guards. Wesley will ask you to look around. There is a Health Upgrade in the back right storage room along with a TON of ammo. Ignore health items for now and load up on ammo. Eventually Wesley will tell you to use the radio to call for information. Starting with the health in the storage area make a bee line for the second row of desks in the main room. There are 2 to 3 health packs at each of the three desks and then 3 more on the back right desk. Grabbing all of these will boost you over 250 and get Health Perk I and II. Now quickly use the radio and enemies will start spawning. While you are still above 100 health kill 5 to get Health Perk III.

Kept trying & failing here on Uber. Does the perk stay unlocked if you go back to chapter select, play it on easy, then go back to the chapter you were on to continue on on Uber? Does that mess up the Uber achievement?

I did this at the end of Chapter 4. After the dog jumps through the window and attacks Annette, and she hides in the cellar with Kessler, collect as much health and armour around the room as possible. This should put you around 400-450 for your health.

Once done, go to one of the windows (beside the main door) and start shooting the Nazis. I used a quick-scoping method, which smashed out a load of headshots. There will be a ton of them in the middle of the road (coming from a Gondola type structure) so you should get this fairly quickly. I got Perk II and III this way without much effort.

The easiest and earliest way to do this is in the prologue right after you are able to use your pistol for the first time you will kill two guys and before talking on the radio run scout around the room for all the health packs, and there is a lot of them, and pick up all the armor you can find so you will lose health slower. Right after you are able to move after talking on the radio you will have some time to pick up all the health packs and this will likely get you the achievements for Health Upgrade I and II. Run in the back room, I recommend using a single assault rifle, and peek your head out and kill 5 guys. Watch out for grenades, It is best to do this on a lower difficulty.

If you decided to play the whole game on Über difficulty and are having troubles being able to find a spot to even make an attempt for this, I found a great one.

In Chapter 6, once
, there's a spot to grab this perk after you've regained your weapons. After you leave the small room that has your weapon's chest, you'll start ascending some stairs and fighting enemies. Try to keep your health at 100 and leave any health packs you see. I recommend the Schockhammer for the 2 dogs that you will encounter. At the top of one of the stairs, there are 2 health packs on the ground next to a chair or plant (or both). The room you'll eventually use to pop this achievement isn't too far away.

Clear all the enemies out, up to and including the room with the 2 holes in the ceiling. You can see the door to the next room through a very tiny connecting hallway. Do not approach that spot yet. There's enough health packs in this room and the few rooms before it to get your health up over 200/250.

Once you've searched these 3 or 4 rooms (there's at least 2 of the +40 packs, one of which is on the ground behind a bed, and a few big armor pieces) to know exactly where the packs are, start your fast-walk (no need to sprint, it'll disorient you) to pick them all up and head for that room through the small connecting hallway. There are around 6 or 7 enemies minimum in here. I used a combination of dual-wield auto rifles and single-wield Schockhammer and got it done pretty easily. I think I still had over 200 health left.

Good luck.

Requirement: Obtain 5 kills in a row whilst your health is overcharged

Your health will be overcharged when you continue to pick up health packs once your health has reached maximum level (100). Your health level will turn orange and it will slowly reduce in number back down to 100. It is during this time that you must obtain 5 kills in a row. This will likely come naturally so do not worry about it too much. A good place to obtain this trophy though is at the end of Chapter 4 - Escape! when defending the Tavern. There are enough health packs in the tavern to overcharge your health to 400+. This will also take several minutes to fall back to 100, and there are plenty of enemies that try and store the tavern in which you can kill.

The Health Upgrade III perk increases BJ’s maximum health points by 25 points.

In order to unlock this perk, you must perform the following requirement at anytime during the Campaign:

  • Kill 5 enemies in a row while overcharged

TIP: If you managed to successfully overcharge your health during the aforementioned area of the Prologue, then continue on with the objective and contact Kessler. After you’re finished speaking with him, you’ll be ambushed. Make sure to take cover immediately and start picking off the enemies that flood the room. There will be more than five enemies to face during this fight, so long as you pick up ammo in the previously mentioned storage unit, you can pop in and out of cover to kill five enemies in a row while your health is still overcharged.

During a surge of strength (health above normal), you need to kill 5 in a row. The same is done in an elementary way in the prologue as soon as you are allowed to shoot, there are enough first aid kits on the tables and shelves to increase your health and there are also plenty of enemies, if it doesn’t work then do a restart

An even more obvious place to get this perk is at the very beginning of the second chapter.
We are sailing on a boat. Almost as soon as you start moving, there will be a balcony on the right with armor and a first aid kit for 75 HP.
We grab it and stand behind the machine gun, there will be about seven opponents immediately ahead.

In the tavern, with more than 500 HP, we do all the health perks here. The third is done when the Krauts begin to climb into the windows.

Easily done in chapters 6-7 on a crowd of zombie Nazis. Did it without even thinking, with the help of one grenade and a shotgun blast.

I got it at the end of the first chapter, when you need to board a boat behind the turret, made both trophies, immediately after clearing the location, collected all the first aid kits and jumped into the boat. 250hp and 5 kilos from the turret without any strain and according to the plot.

The easiest way is in the tavern. When you come with a girl to a man, in this room there will be a lot of first aid kits and other things that increase the health of everything, you can increase your health by more than 450 units. And after you hide people in the basement, the Krauts will start to come to you, there will be more than enough of them there and you will also have plenty of time in a surge of strength.
