You have to complete all 4 of the base game's episodes on any difficulty. The episodes are New York, Jerusalem, Moscow and Tokyo.
DLCs are NOT needed.
You have to play each chapter online with friends / bots. Personally I used Online (private) and played only with bots on easy.
Do not join a game in a session, you have to start each chapter from the start.
