If you clear all 14 missions on Insane and the achievement doesn't unlock, simply restart your game and the problem will resolve itself.

I think some levels may actually be easier with fewer people and then others are easier with a full team. Me and a friend just finished this after going at it for awhile, always matching with two other randoms that were frustrating to play with most of the time. One level that was really hard and giving us trouble was Setting Sun, the first level of Tokyo. Eventually the game was kind of dead there so it was just me, him, and two bots. Literally the first try we did with just me and him we beat the level fairly easily, the defense sections even felt easier. Next level again first try with just me and him. The final level however was a different story, we couldn't get past the part with the two doors you have to close, it was way too hard. So we set it to a public game, managed to get two more people and then breezed through that part. So some levels seem easier with fewer people, and then others easier with a full team.

Well, I had the achievement done already and was with someone who needed Jerusalum 2, Tokyo 1 and Tokyo 2 from launch version of the game. We finished those base Missions and achievement didn't unlock until we did the Tokyo 3 June Update Mission, and New York/Moscow 4 October Update Missions. This was all done with 2 bots and 2 live players, I assume it could be done solo.

New York 2 I am struggling with.
The end train part protecting the Driver. They seem to get him too quick and bots don’t help enough to save him, just fire at any old zombie..

Did it all with bots, only struggled on Moscow "resurrection" at the end in the train station, those service rooms are your best friend, but don't spend to much time in them or you run out of ammo.

Anyone having trouble with this unlocking. When i look down my list its showing they are all completed on insane but not achievement. All the new ones done as well dead in the water was the last one i did.

Cancel i reset the gane and once i signed into the servers again it popped.

New York 4 is the real tough spot now. We had way more trouble with that, even with the modifiers on. Unfortunately we didn't get it done in time, and it doesn't mark it as done.

That patch sucks balls, we finished a few more levels before realising progress wasn't being saved any longer. Really killed off the fun for us now, it's only possible with other decent players, the AI is useless.

My thinking would be if you had the game installed and the mutator update but didn't download the newest patch the method would still work it would just have to be played offline. Sadly I didn't have the game before the 11/20 patch or I'd try it out for myself.

Finally someone confirmed it :) Do you know if it works only with bots too? Also beating it on extreme unlocks skins so that counts too.
A good and fun combination is Thief mutator with a fully upgraded Slasher.

As far as I'm aware, you can't work around it since the mutators weren't in the game at launch. If you delete the game and reinstall it offline with a disk, chances are that it'll just be the launch version. I have no way to test it though since I bought it digitally.

This patch crap pisses me off to no end! Just checked the PS4 guide for this cheev, and it says specifically that it is still possible for PS owners to play with mutators even after the November patch/update... so PS users can breeze through the Insane run while Xbox users have to struggle/give up... bastard devs!!!!! >:-(

Some tips and interesting facts about this achievement:

-Unlike Left 4 Dead, you don't need to be alive when survivors escape. If you are dead and your team escape, it counts to you too.

-You can join a game that it's already started. Is not necessary to start the game from the lobby.

-An obvious tip: try to play with people you can communicate with. If you play with random people, even though it's possible to pass the game, you will probably fail. I did in in a party with my brother playing with 2 cool french guys that were in their own party. But we were sending each other some messages some times. If you play in pairs, like I did,but cooperating the 4 of you, the game can be pretty easy.

-When people are shooting in a closed place, if you are in the front line, always coach down to let the people behind you to shoot and to avoid friendly fire (which can be deadly in insane).

-About classes. 2 medics and 2 fixers at level 30 should me more than enough. The perks to use are very subjetive, each one could prefer different perks, but if you want to know the ones we used, let me know. Obviously, use the class wisely. First aid kits should be always for medics and things like that. This is one of the biggest problems playing with random people.

-Stealth / Speed run. Use silencers to pass through some level parts, it makes some parts way easier. Other option is to use fixers grenade to became undetectable for a while and be able to pass parts in just seconds.

- Defending objetives. One of the worst thing playing online with random people is that they tend to forget about objetives and they only focus to stay alive. There is no sense on staying alive if zombies will take the objetive. In extreme situations, give your own life (or deffend it with masked effect) to deffend the objetive. If that makes you to pass the chapter, it's worth it (remember that you don't need to be alive). But if the 4 survivors play good, this is not neccesary.

-About weapons, you can use the one you prefer. Bit be sure you got the highest version of it. And try to avoid melee combat as much as you can.

-Don't be impulsive or impatient. Stay calm and move slowly.

-Learn where special zombies respawn. After some games played, you can learn it and it can be very useful.

The more difficult chapters for me were the second one of Moscow, the first and second one of Jerusalem and the first one of Tokyo. But after some attempts you start doing it better until you finally pass the chapter.

Just finished this off today playing in random lobbies and wanted to say that it's possible to do it that way if you can't find anybody on TA. There's a lot of people playing on the harder difficulties that have no business playing on them but there's not much that can be done about that. In my experiences, you either need to play as the Medic or Fixer class and it's vital that any perk that either grants the Masking Grenade effect or enhances it in anyway is applied. If you're a decent player, have your class and weapons maxed out, you'll eventually come across a lobby with competent teammates and should be able to make it work. It's possible to do even with a weak link (low level, un-silenced weapons, complete incompetence) if at least 3 people know what they're doing.

Recommend players stay close, to always keep track of team-players. I've seen way too often where 1 player falls far too back, the team try's to go back and end's up failing the mission or the lone wolfer decides to rush ahead while the team is barely able to keep up. Also press up the 'd-pad' helps people spawn special infected, which is helpful on Insane.

@motorexGTR32 , the achievement was originally labeled hardest. because the hardest was insane. but with extreme coming soon. they changed the title. so it wouldn't confuse players.

Just a heads up since the new update they’ve made the zombies a lot more on point! Tokyo mission 1 is near enough impossible to beat, had a spot on team all max lvl and the best perks you can use and we got wrecked every time. If anyone wants to try later add me GT: Juicy Plumbs

On the last level, Cruise Control (Tokyo #3), there is a glitch at the last defense point where you can run up a little blue chair and then walk on the fence/ramp and stay there until the chapter is done. Search on youtube for "world war z cruise control glitch" to see what I'm talking about. It was really helpful !

The Aftermath update significantly increased the special infected spawn rates which means that the #1 threat is getting pinned down and subsequently killed by one of the many, many disablers. As such, you need tools to deal with them whether offensive or defensive. Secondarily, you want to think about self-sustainability. Pick perks that give temp health, a free self-res, and/or weapons that proc healing on multi kill. Lastly, consider what type of team buffs each class brings, e.g. Exterminator's bandolier perk for 25% improved defenses. These effects stack if you run multiples of the same class!

My top tier classes:
Slasher / Dronemaster / Exterminator

Mid tier:
Hellraiser / Fixer / Medic / Gunslinger

Bottom tier:
Vanguard

walk, don't run. In a recent update they increase the size of the swarm depending the amount of noise made by running (the idea is your noise draws in more zombies from further away depending on your noise level).

My biggest issue with matchmaking for this achievement is the number of low level people that are qued as well as those that don't have silenced weapons. Having to grind thru insane constantly fighting is awful.
Finished this last night. I'd recommend at first just quick queuing insane and see what you can jump into. After that select the maps you want and wait for your lobby to fill. If people aren't lvl 30 (or close) or below 50 id caution even starting as they will likely not have silenced weapons and your chances of success will be greatly diminished.
Despite finishing this achievement I am still happy to help with other maps as long as you got the levels and the weapons.

I've added all of my videos to this solution which are without mutators and on Extreme difficulty. Good luck and I hope my videos help you.

In short you have to complete every level in the game on insane difficulty whether you go solo (not recommended) with bots or online with other players. You can also play private matches with friends rather than public and it still counts however you cannot use MUTATORS.
This will void your tally for completing it but you will still get the full amount of coins dependant on the mutators you use, just not the completion of the level.
You dont have be alive at the end for it to count just as long as someone survives and finishes it.

The game will track which levels you've done with the red skulls under each levels picture.

I highly recommend playing through the story a couple of times so you get an idea of what the levels are about and where to go. Then focus on ranking up your classes to 30. The fixer, medic, exterminator and hellraiser are the best classes I think but everyone plays better in their own unique classes so give them all a try and find which one you prefer. Also any weapons you like using its worth grinding the kills to get the max upgrade for each weapon. The advanced smg, assault carbine and shotgun are by far my favourite weapons and cause carnage when upgraded fully.

Stick together, play stealthily and above all
ALWAYS USE SILENCERS.
In my Moscow 1 and 2 videos one of my pals was using an unsilenced weapon throughout and we got pretty much endless hordes, we thought it was a bug but it was only after we finished both levels he was like oops XD

The classes all have their own unique perks, the medic is handy for stims, healing properties and having masking on stim.

The exterminator is great for crowd control especially with the RPG, also you get a 20% chance of getting a duplicate defense kits which is a life saver on finalies.

The fixer can start with a breach and use masking grenades.

The hellraiser is great for claymores, can also start with a breach and a beasty MGL also their explosive perks are awesome.

Special infected:

Lurker/hider - listen out for these buggers they make a very distinctive noise kind of like a gurgling sound when they're nearby so check your corners when proceeding. All specials pretty much have random spawns but the lurker sometimes turns up in similar spots.

Infecter - easy to take care of just try to strafe to dodge her spit and aim for the head, as soon as she goes down on her knees go for the melee to instakill.

Bull - when he charges, sprint to the left or right or if there are any large obstacles try to run behind them, sometimes he runs straight around corners so keep that in mind. His weak spot is his back so once he's charged go for the melee or shoot either way.

Gasbag - easy to take care of just aim for the head and watch out for the radioactive cloud, if it blocks your way just wait for it to dissipate.

Bomber - not much to say for this one really just listen for the ticking sound and when you see him shoot. If he gets too close you can melee him to knock him back.
Also if your brave you can shoot him in the legs and melee him on the floor this will give you random loot. Risky though.

This is unbelievably easier on insane as you only actually need to turn on one electrical grid in the finalie and it's pretty much over. On extreme you have to do 3!!
Anywho I think this has to be one of the hardest levels in the game due to the finalie.

If you want, you can pretty much run through the first part to the first safe room but make good use of masking grenades.

The second part on the bridge you just have to watch out for claymores and make sure you pick off any z's before they spot you. If you get through sneakily and jump down you can pretty much run to the elevator too. In the car park you'll either get claymores or auto turrets so beware.

If you trigger the swarm on the bridge pop a masking and run. Once at the car park go into the room on your right when you reach the bottom of the ramp and close yourself in. As long as you dont let too many zombies push the door for too long it won't burst open so you can easily pick off the horde. Just beware infecter can spit at you.

Right the hardest part. Basically you have to defend 3 parts of the hospital - the front, left and right wards and potentially a 4th which is by the cliffs.

I recommend putting an auto turret in each ward one on the left and one on the right. If you manage to get a 3rd put it in front of the main hospital doors. When playing this level I normally have one person in the left ward by the auto turret and same in the right. The other two stay on the top left upstairs in the room with the hospital beds overlooking the outside. Preferably one of them with the flaregun.

If you get in trouble up there you can double back to the left or right ward with the autoturrets.

I recommend having subtitles on as once you've defended you get told over the radio that you need to restore power to the electrical grid in a certain area which can be one of 3 places, the morgue, upstairs in the left side of the hospital or downstairs also on the left side of the hospital.

It's best to save a masking grenade or stim shot so you and any comrades still alive can together make a break for the button as the swarm will just keep coming until the grid is turned back on.

After you pull the switch its either the end of the level and you have to go back to the the front of the hospital or you have to make a break for the cliff entrance and defend for the last time.

Luckily on insane it will show you where you have to go so you know which buttons to press etc.

You can no longer do this with "mutations"

The only way you can unlock this now is a full team of super dedicated badasses, who know the game off by heart and never make mistakes

Couple years late, but would just like to say you don't need a full team of super dedicated badasses. Me and my friend did this together, no mutations didn't even know that was a thing. Some levels we did with two bots and sometimes we did it with two randoms, it seemed like some levels were easier with bots and others weren't or we just got lucky I have no clue. We were definitely not badasses though that never made mistakes or anything, there was definitely a lot of trial and error and just luck with items. Like the tier 3 shotgun if I remember correctly is a god send if you can manage to get that weapon. It is for sure challenging as hell, but not impossible if you aren't all badasses.

It is not necessary to go through the chapter from the very beginning; if you connected with someone in the middle of the game and completed it, it also counts.

Episode "Moscow" chapter 1, easier to do solo with bots. In general, the more living players, the angrier the zombies. But some chapters can only be completed with a good team.

Completed the 1st mission in Tokyo for madness, the trophy did not fall, perhaps a bug or after the update you need to complete new tasks From the dlc

I completed several missions alone because I was tired of dying from friendly fire. In fact, the strength and number of monsters depends on the number of living players in the party. The following classes are suitable for this trophy:

Technician 2 pcs. (through camouflage grenades with an additional camouflage effect, healthy lifestyle, penetrating charge)

Medic 1 pc. (through a skill that gives a 25% chance that the first aid kit will not be spent, healthy lifestyle)

Ripper 1 pc. (via chainsaw)

The whole point of the passage is to quickly and quietly pass, simultaneously looking for first aid kits and giving them to the medic, giving the equipment to the technicians and the ripper. Kill zombies with a precise shot to the head; if you start killing everyone at once, they will start making noise, and this will respawn the wave. You can’t wait in one place for a long time - the game will respawn the wave. Do everything quickly and quietly; if there is a wave or you have reached the point of clearing the horde, then camouflage grenades and the ripper’s chainsaw go into battle. Taran is calmly killed by the ripper, using a taser and going behind his back. If the screamer respawns, we quickly kill him. It is better to place a penetrating charge on the last defense; if a penetrating charge is placed, then it is noise and it is a wave again.

In September there was a big update that added 2 new maps, New York 4 and Moscow 4, they also need to be completed.

If you plan to play on the "madness" difficulty with randoms, then first fully upgrade the medic class to level 30. On weekends there are a lot of skilled PC players (level 999) playing and it’s much easier to pass difficult maps with them. Your task is to play as a medic and be the maximum support for the team. Give a friend temporary health from a stimulant pistol. Always grab the chainsaw as a heavy weapon. An injection for temporary health and a chainsaw in your hands allows you to easily restrain a large number of zombies alone without harm from the friendlier.

After patch 1.05 where a new level for Tokyo was added for the first time.
The trophy has changed slightly in terms of completing all levels on Insanity difficulty.
Now you definitely need to complete 14 missions instead of 11 on “Madness” or “Extreme”.
