Start by creating a new superstar.
On the first page it gives you 3 options for a head
1. Superstar
2. Diva
3. WWE Superstar
Select WWE Superstar, then select faces. Choose which superstar face you want, hit the A button and then use the Right Bumper button to scroll over and finish editing your custom superstar.

Go to the Creations part of the game, and create a new Custom Superstar. You'll be given the option to choose a WWE Superstar head such as CM Punk, Daniel Bryan, etc. Choose one and save the Custom Superstar for the trophy.

In Create a Superstar, select Head Templets, then WWE Superstars and pick any head. Confirm the creation and achievement unlocked!
