How to unlock the The Celtic Warrior achievement in WWE 2K16 - Definitive Guide
Follow the guides for the "Leave Me Alone" achievement for causing a critical injury. Once you've inflicted a serious injury on someone, simply simulate a few months ahead on the calendar and your superstar should be fully healed and should get the achievement during the simulation. If you don't have the achievement by the time it's finished simulating, simply simulate a few months further. It took me 4 or so months of simulating for mine to fully heal.

The thing that happened to me was that I had a critically injured superstar but when he fully healed I didn't get the achievement

After inflicting a Critical Injury on a wrester and popping the following achievement:
Its now to to recover your poor victims health.
Go into EDT SUPERSTARS, Select the Wrestler and select STATUS EFFECTS. Check that the Critical Injury is Active
Usually it takes 1 month to heal from a Critical Injury to Full health.
Select Play Matches, Press Y for Calendar, Select any event 1 - 2 months in the future. Press A on the Event. (An Attention bar will appear saying that The Matches up to the selected date will be simulated, Select Yes)

Have a created custom superstar and edit their attributes to zero for head, arm, body, and leg durability, as well as damage recovery. Save it and then go start playing matches in WWE Universe. You'll have to edit matches and add in the custom superstar and whoever you want to face him with. I would suggest doing a one on one match, making it either an Extreme Rules, TLC, or No Holds Barred match so you can use weapons.
Target any of the limbs you want, with the head being the easiest. Continue to use moves and weapons that target that area and get the body part to red. You can do this for multiple body parts as well. Once you have done enough damage, win the match. Then move onto the next show and do the same. Rinse and repeat until he is out with a critical injury. You can check the status by going into "Customize Universe" and selecting "Edit Superstars". Go to your created superstar and it will show it under status effects.
Once he is critically injured, just simulate a month or so forward and the trophy should pop for you as he should have recovered. If not, simulate further until it does.
