Just 2 add-on to the other solutions.
I was in a feud by RR so I couldn't be in the RR match.
I basically stood in the WHC picture until MITB(Ranked #5, not sure if it matters) stayed out of any feuds for the month of June and got in the MITB ladder match won it then the following night cashed in the briefcase and won.
The cash-in match must be on a normal match with the current champion involved.
Just press
like you would to run-in on a match and you'll get an option to cash-in or just do a regular run-in. Your opponent will have no health, so it's pretty easy after that. You might have to perform a couple of finishers depending on who the champ is(Lesnar,Cena) achievement pops during celebration.

There are two ways you can do this you can either just fight for the world title until you become number 1 contender or if your close to the royale rumble just make sure you are contending for the world title and have no rivalrys and at the royale rumble you will be in the royal rumble and then just win it and face the world champ at wrestlemania (if someone attacks you ignore them don't start a rivalry)

This may still take a while to do, but I've found a way to cut out a lot of the grinding. As NoahMLP said, the easiest way to get this is to win the Royal Rumble, and beat the Champion at Wrestlemania.
However this may take a long time, and sometimes it's difficult to avoid Rivalries, that will cause you to miss out on the Rumble.
What you want to do is, change the title you're after, so you're obviously going for the WHC, what to do is choose the IC title as your title, and it'll advance a show, then choose the WHC again, and keep doing that until you get to the rumble.
*MAKE SURE YOU'VE SELECTED THE WHC BEFORE THE RUMBLE PPV*
Set the difficulty to Easy, and win the Royal Rumble.
Then you only have to play 2 months in game time to fight the champion at Mania.
Any problems, please let me know.

Winning Royal Rumble doesn't work.

As of patch 1.05 winning the Royal Rumble does put you in the title match at Wrestlemania - in my case a triple threat with the champion Brock Lesner and The Rock.

To win the WWE Championship, you need to be rank 3 of the WWE title division and locked into a feud with the champion. The championship match should take place at the pay per view.

Alternatively, you can win the Royal Rumble or Money in the Bank. To get entered in either match, you can't be locked into a feud. However, I won the Royal Rumble but was never granted a title shot. I've heard reports that you still have to be a top contender for the title.
When I was rank 4 I won Money in the Bank and beat Brock Lesnar the next show for the WWE title and the achievement.
UPDATE: According to b6nji winning the Royal Rumble works properly now. However, I've seen reports on the 2k forums that it still doesn't. Someone on that site said it worked for him. I think it's buggy at best.

The tutorial will tell you how to go for championships. You will go to the ranking system and it will list each championship you can win. You will be put into the hunt for the NXT Championship automatically. Continue to rise through the ranks by competing in matches and getting into feuds with other superstars. You will also want to do run-ins on the title holder to get a fued going with them when you reach rank 3. If you try earlier than that, they are basically unbeatable as they reverse everything. Eventually you will get your opportunity at the title. Once you have won, go back into the ranking system and go to the next championship in the line and press 
to join in the hunt for it. Do the same as previous to get that belt. Do this for each one.
For the tag team titles, you will need to be on the good side of a superstar to have as a partner. Make sure you are doing run ins for a superstar to build up his relationship with you, otherwise you'll have no one to tag with.
