Similar to all of the other Payback achievements, all you need to do is head over to the PLAY mode, select ONE ON ONE, select a match, select a wrestler, hit EDIT PAYBACK and make sure they have the red major POISON MIST payback equipped. Now load up the match and let your opponent beat you till the payback meter fills up. Once it is fully charged, simply press 
+ 
to start your Payback countdown and then press + again while your opponent is in front of you to use your payback and the achievement should pop.
Note: Some of these achievements may not pop up the first time and may require a second try.

Start a play now match as Papa Shango. Let your opponent beat you up until your red payback meter is full, then go on the offensive and press 
+
to activate the payback. Once you have the opponent groggy press + again to use the poison mist on your opponent. Note this will get you disqualified if you are in a normal match.
