This is storyline driven, once you go through the story gate.

The story is "This is why brutality" and you will enter another story gate called "Who's Dom's Dom". I would recommend playing everything before this (yes, it's long and tedious) because other achievements rely on this.

The storyline will have you either trusting (or not) Rhea Ripley and Dom will tell you not to trust her. I don't think your choice matters since it's logical she will turn on you either way. This is typically predictable of WWE2K games anyway.

This achievement is at the end of "Dom's Dom" where you will be in a very long ambulance match with a lot of button mashing. I recommend the entire body goes to red before you even try putting Rhea in the ambulance. Even all red didn't help as much as you think.

Once Rhea is in the ambulance and you are off celebrating, the achievement will pop.
