For this achievement make sure you select an extreme rules match or a no holds barred match so you don’t get disqualified.
During the match you can select objects under the ring if you click
and select any weapon. After go near your opponent and click 
on your controller and your wrestler should throw the object at your opponent. The achievement should pop soon after!

Done this many times and it still wont pop up. Maybe I have a glitch or something

You need to be playing on normal difficulty to earn this trophy
Pick a extreme rules match and then pick your wrestlers and then grab a weapon from under the Ring once you do this go near the other wrestler a press circle when holding if done right you should throw and hit your opponent earning you the trophy

Seems like it has to be done in an exhibition match. I thought it was glitched for me as I've thrown weapons multiple times in other game modes and never got the achievement. Now I was aiming for the "Closed Casket" achievement and when I threw a weapon at the Undertaker it unlocked. And the decription of the achievement (at least in the German version that I am playing) says "Exhibition vs. AI" in the brackets.
