Access hidden menus, on main menu, highlight options and enter the following:
Cheat Menu: L+Left+Y+Start
Backer Menu: L+Right+Y+Start
Within Backer Menu select Backer 7 "Jacqui" and select Backer Area as "The HQ"
Within Cheats Menu turn on Infinite Health, Infinite Ammo, Infinite Grenades, and select the starting area as "THE HQ".
Start game and enter all doors either to the left or right, first before entering doors at the top. Complete level and rescue all hostages you see.

Left D Pad Also works, in case anyone else is confused and doesnt have 2 fingers more on his hand.

The update as of 2/13/2020 has fixed the broken achev.
Take your time and check each room from right to left on each area before moving into any north rooms.
The achev will pop when you enter the room for the last boss fight.

I cheesed this one quite nicely using the previously mentioned cheats.
What I did, was start on the Forest level, and by the time I reached the end of HQ level, I had rescued all hostages, and the achievement popped. I had previously reached Forest level, and rescued them all in a previous run, so not 100% sure if those counted. Either way, this may help you if you are having issues.
