Yakuza Kiwami

This sub-story is a bit of a pain because it requires you to do this in multiple chapters and will be un-finishable until late in the game. This side-story starts in chapter 6 when you see a kid next to the entrance of purgatory. find out he is hungry and go get him some food. After you give the food to him go to public park 3 where he will be waiting for his mother. VERY IMPORTANT!!!! Talk to the boy BEFORE you take Haruka around town in chapter 7 because it will not let you progress. This is annoying because the next step needs Haruka to complete. Talking to the boy reveals he is lonely and Kiryu suggests Haruka to relieve his boredom. If you do not complete this story in chapter 7 you must wait until chapter 11 to complete. This is annoying because the other two side-stories with achievements will not start until after you finish this one. Anyway, after you take Haruka to him he will be hungry again. Go to the nearby store and buy more food. Then after giving the food to the boy he will collapse. Go down Taihei Boulevard West and you will see a building with an anime nurse on the front of the building. Watch the cut-scene and afterwards fight the two guys. they have big health bars but should be easy at your level. once you beat the guys watch the remaining cut-scene and the achievement will unlock.
