The battle has 3 phases. First is the tentacle sweep. Just jump over it to avoid damage. After a few swipes, it will smack down in the center, so just jump to avoid the shockwave. The eyes will now be open, so hit them! The small eye requires 1 hit to close, the middle needs 2, and the big one won't shut until hit 3 times. Next, a tentacle attacks from above. Just watch the shadow and jump when it stops to avoid the shockwave. Then, an eyehole opens for you to smack on a few times.
This repeats twice more with slight variation. Starting with the second go around, the area will be flooded, so your jumps aren't as responsive. Also, a new attack happens after the tentacle sweep. A tentacle will attack from the water, so just swim and jump in circles to avoid being caught. During the last round, the attacks from above and below will happen simultaneously.

In order to beat the World 3 boss, Trev, you first have to beat one of his tentacles. Jump over the tentacle whenever it sweeps through the room, then dodge the slam attack afterwards. Pay attention to the shockwave it sends out as this will also damage you. Once he has slammed it onto the ground, 3 eyes on the tentacle will open that you have to attack. The smallest one needs one hit, the middle one 2 and the largest one you will have to hit 3 times before it closes again. If you're quick enough, you can close all three eyes within a single phase.
After all 3 eyes are closed, you will have to dodge repeated slam attacks and shockwaves. This time though, you will only see the shadow of the tentacle that follows you around. Eventually it will stop moving which means the attack is imminent. Eventually a giant eye will appear in the back of the room that you have to attack.
After you have inflicted some damage to the giant eye, it disappears and the room fills with liquid. Additionally you will be chased by a tentacle that you will have to dodge. This time it's not slam attacks though but simple contact with the tentacle that you have to worry about. Jump away whenever it gets close until it disappears below the surface. Now, the fight goes through the same phases again so start from the beginning until you've beaten Trev.
