Complete all 5 levels on Insane to earn this trophy.

This isn't quite so bad as it sounds, as you have infinite continues and the game has generously placed checkpoints. Even if you do lose all 3 lives and have to continue, the enemies you've killed remain dead, making this more of a tedious grind than a difficult task (aside from the last level).
