You have to complete every duel and reverse duel in the campaign. You do not have to use the story decks you can make your own. To speed it up I recommend creating an exodia deck by buying grandpa Moto’s card pack.

This achievement bugged on me. I was 100% in this campaign. but it didn't pop. 100% everything. Did multiple fights in this again just in case. Deleted my save and started from scratch and still didnt pop after getting back exodia deck and then doing all of 5d campaign then all the reverse duels. I ended up deleting my save again and playing on an alternate xbox. In one sitting I got my exodia deck built again then did all the 5d this time doing main fight followed by the reverse and than main/reverse etc. This achievement popped after finishing the final main fight before doing the last reverse.

I did not have to do any reverse battles. I did need to delete my save the first time though because it glitched the first time
