To unlock all the avatars, you have to beat everyone in every campaign on every occasion. This includes beating characters you played as in reverse duels. The protagonists may be an exception to this as you can use their avatars from the start
You can check what characters your missing by going into Challenge mode and looking at the characters still blacked out in each series.

You still have to do all of them reversed to unlock all of the challenges.

I realised that but neglected to add it in to this solution because you can get the avatars quite a while before you can do all challenges. added it now though.
