How to unlock the Chance of Loss is Not Zero achievement in Zero Escape: The Nonary Games - Definitive Guide
This unlocks when reaching the ‘Safe’ ending in 999.
The only way to trigger this ending is to go through door number 5, then 8, then 6.
After going through door number 5, complete the entire puzzle, but don't leave yet (thanks to I8ITackyticsI8I for elaborating)! Look at the safe you find in one of the closets to trigger some dialogue and find out that you can't open it (yet). Check it two or three times to make sure the correct dialogue is triggered. You will return to the safe automatically very late in that run.
You cannot trigger the required dialogue as long as you haven't finished the puzzle yet.
Also, triggering this ending is a prerequisite for unlocking the true ending.
If this is not your first play-through, you can speed up previously seen cutscenes by pressing cn_X until it says ‘skip’ in the bottom right corner. If you have not seen that particular cutscene before, pressing cn_X will only cycle between ‘stop’ and ‘auto’. The 'skip' mode will stop when player input is required (for dialogue options or choosing doors).

You can enable skip in the options and skip unseen text.

This description isn't really detailed enough. The important things to note
- you have to look at the safe AFTER completing the puzzle to door 5 to unlock the exit and before leaving. If you look at it before this it won't count as you won't trigger the right dialogue.
You also need to make sure that you have the key unlocked on the flow diagram along the path - if you've played before skip to the next key or the next door to make sure you make the right choice. If you've done the puzzle before for a key square, then it will bring up the dialogue options to unlock the key - you will know if you've done it right if the leave no one behind square changed from a locked padlock to an unlocked one. You can then select from leave no one behind to see the safe ending.

The Zero Lost ending (otherwise known as the Safe Ending) is one of the unique endings to 999. It is required in order to unlock the game's real ending. You do not need to worry about any dialogue choices but you must choose the following door order when given a choice: 5 / 8 / 6.
