Revive 5 times in Coop.
In order to do this, you must be in COOP. Currently there is only LOCAL COOP on this game. It is a little hard to get into, but essentially you must have the second controller plugged in / powered up when the game loads. Then when you start a new game, the second player then joins.
When you fall, run to the player *after you killed the remaining enemies because they seem to interrupt your revive and HOLD

I got this after I revived 2 and my second controller revived 3. Don't know if that was a glitch or not, since many of these achievements are in fact glitched.

When playing co-op, if your companion runs out of health he will be knocked out. Simply run over to them and hold 
for a few seconds to revive them. Do this five times and the achievement will unlock.
