This achievement requires a little luck. When you level up, you get to chose one of two cards to give yourself a perk. (If you've chosen the perk to show more cards, then obviously you will have more to choose from.)
You need to choose the same perk five times in one game to pop this. That's where luck is involved. It took me a few playthroughs to have the same card show up enough times.
In the beginning choose the cards you like. As you play, you will see some repeat themselves. Always choose the card with the highest number in the top right corner. When you finally see one with a five, select it and "blup" you have it.
Some unlockable characters start with a level five perk. Those don't count.
