This achievement like so many in the game is glitchy , I have done this tuns of times and it has not popped for me.

LUL what is this here? Followed the video guide and I ain't getting this trophy but got two instead...Cranial blowout and Chain reaction...

Throughout the game zombies will resurrect if you don't badly dismember them OR shoot them in the head. When one of the zombies resurrects make sure to get a headshot on it.
