At the very ending of Chapter "Purgatory" from Episode 2, you come to a train station, in front of you, you see a baby carriage, interact with it.

You can get this in the first mission of episode 3. The mission is called "City of Ashes".
You'll come by some baby beds in the Hospital. Go to the one in the left corner where you can interact with a doll.
After some feasting on flesh this achievement will pop!

I unlocked this achievement at the end of the level Purgatory. After a safe zone, you will see a baby carriage on your way. Just push 
button when you are on front of it. The achievement will be unlocked immediately.

You can get this in Episode 3, Chapter 1, 'City of Ashes'. After entering the hospital area, you eventually come to some baby cribs. Crib #1 has a doll on it. Simply use it and watch what happens and get the achievement.
