Upon reaching the farm, choose either option to get rid of the old people. Now, go to the main room and you'll Leo to interact with the banjo and Vincent to interact with the piano. Select the easy difficulty and have both characters play about 15 notes in a row successfully to trigger a cutscene. Achievement unlocks.

This video guide contains all achievements / trophies in A Way Out in chronological order. Nothing is missable, you can redo anything via chapter select and unless written otherwise, both players earn the achievement / trophy at the same time (They can only be earned with a full copy of the game, not the free friend pass). This game can only be played with 2 players, there is no Singleplayer mode.
#1 The Dip - 0:25
Chapter: Escape - The Yard
Leo must do 20 Dips in a row on the prison yard.
#2 Freedom - 1:29
Chapter: Escape - Prison Escape
Free the Caged Parrot in the Warden's Office.
#3 Take a Breather - 2:49
Chapter: Fugitives - Breather
At the start of the chapter walk to the right. There's a rock both players need to sit on to enjoy the view.
#4 In Sync - 3:49
Chapter: Fugitives - The Farmstead
Go into the house of the old couple. Vincent must play the piano and Leo the Banjo. Each player must hit 20 notes in a row without missing a single one. If one player messes up you have to get another 20 correct notes.
#5 Timeless Treasure - 5:11
Chapter: Fugitives - The Farmstead
Go into the house of the old couple. On the ground floor is a clock, both players need to interact with it at the same time. This opens a secret door upstairs, go into that hidden room to pick up a sword from the chest.

Chapter: Fugitives - The Farmstead
Climb the windmill and interact with it a few times to spin it up.

Chapter: Preperation - Reunion
Talk to the woman with the baseball bat in the starting area of the chapter. Then go to the trailer to her right side to find her husband. Then talk to her again to get the trophy.

Both players need to sit on the swing set at the same time.
#9 Home Run - 9:16
Chapter: Preperation - Reunion
At start of chapter, head to the right. There's a guy with a baseball bat. Talk to him and then try to hit a Home Run. Swing the bat as soon as the pitcher starts the throw-animation. You must hit the ball in a straight line over the fence really far away. Only unlocks for the player getting the home run, the 2nd player has to do it again.

Chapter: Preperation - Reunion
Vincent must bring a Wrench to the woman repairing a bike. Then help her start the bike. Leo should stay inside the trailer until the trophy pops and not progress any further (unlocks for both players).
#11 Managed Anger - 11:23
Chapter: Preperation - Violent Questioning
Talk to the guy on the chair and choose to push back the chair. Leo must then pick the purse from the table, drop it on the floor, and lean over the table. Vincent must then use an object to torture the guy which triggers a dream sequence.
#12 Live the Dream - 12:58
Chapter: Preperation - A Nie Life
After riding the elevator both players must sit on the couch in front of the TV. Watch TV until Leo falls asleep.

Both players need to play the arcade at start of chapter.

Leo has to interact with the Helicopter at start of chapter.

While inside the old couples house, you will come across a Banjo and a Piano downstairs by the kitchen table. You will need Vincent to play the Piano while Leo plays the Banjo. Select easy difficulty and hit the buttons as they enter the circle until a cutscene plays and the achievement/trophy will unlock.

After you deal with the old people at the house you can get this achievement. Go to the living room and then up to the instruments. Make Leo play the banjo and Vincent play the piano. After a little bit of playing a cut scene will start and the achievement should be yours.

Al llegar a la granja tendremos dos opciones, seguir el consejo de Leo y capturar a la pareja que ahí vive u optar por la opción de Vincent para que ésta abandone la casa. Tenemos que hacer una de las dos, es indiferente, y el salón de la casa quedará despejado par que podamos explorarlo libremente. Ahí, muy próximo al lugar donde está la pareja durante la cinemática, encontraremos un banjo y un piano. Tanto Leo como Vincent deberán tocar los instrumentos a la vez completando con éxito el minijuego para desbloquear el logro.
When we arrive at the farm we will have two options, follow Leo's advice and capture the couple who lives there or opt for Vincent's option so that they leave the house. We have to do one of the two, it doesn't matter, and the living room of the house will be clear so we can explore it freely. There, very close to the place where the couple is during the cinematic, we will find a banjo and a piano. Both Leo and Vincent must play the instruments at the same time by successfully completing the minigame to unlock the achievement.

Once your in the old couples house, you will see a banjo and a piano next to each other. Have Leo play the banjo and Vincent play the piano, select easy difficulty and follow the button prompts. Once you hit enough of them in succession, a cutscene will start unlocking the trophy for both players.

Trophy unlocks for both players? Yes
This is trophy can be done after making a choice to either do Vincent's way of releasing the horses or Leo's way of subduing the couple by tying them up. The choice you make won't matter for the trophy, so pick whichever choice you want during your playthrough. After completing the choice that you've selected, you should have the following objectives:
- Change clothes
- Get to truck
Ignore the above objectives and do the all the trophy requirements on this farm.
For this trophy, enter the couples' house and you'll find a banjo and a piano in the living room. You need to have Leo play the Banjo nearby the window and Vincent needs to play the Piano. When you see the difficulty option come up, select "Easy" to make this trophy much easier for yourself. This also applies to your co-op partner. Now both of you will get random sequences of

. You both need to play the musical instruments at the same time and try to land about 15-20 perfect notes without messing up. If you've both done this correctly, a brief cutscene will happen and both men will play a song together. The trophy will unlock when the cutscene ends. Please see the Timeless Treasure 
trophy next before you proceed with the current story objective.
If you've fixed the truck to trigger the getaway section, then you've gone too far and will need to use Chapter Select.

This unlocks during the chapter ‘Fugitives: The Farmstead’.
After taking care of the old couple (either subduing them or distracting them with the horses), enter the house. To the left of the staircase (and right of the solarium) is a piano and to its left is a banjo resting against the wall. Vincent needs to interact with the piano and Leo the banjo.
This is like a Dance Dance Revolution mini-game where you have to press the correct button (
, 
or 
) when it passes through the circle. You both need to correctly press the buttons about 20 times in a row and at the same time to trigger a cinematic.
When you pick up the instruments, you have the option to play at a certain difficulty. To make this easy on yourself, select ‘Easy’. Then successfully press the appropriate buttons. When both of you have 20 correct presses at the same time, a short cutscene will play with the two characters playing a song. When it’s over, the achievement will unlock.
Note: As long as you don’t complete the objectives, you have unlimited time to do this. You can also unlock both “Take It For A Spin” and “Timeless Treasure” in this same scene.
