You get this achievement for killing 3 enemies with one grenade.
Not too complicated, you can easily throw a grenade at a group of enemies.
The best place to get this, I found, is in the chapter Salvation, part 20 - Open Season, in the graveyard of the church. Cars will come driving toward you and stop, each carrying 3-4 passangers. Throw a grenade at the car as it stops and you should get 3 kills without a problem.

I got this at the very start of chapter 12, Fuel for the Fire.
Exit the building by going left and make your way across the road to the caravans. Hide behind the tyres, facing the enemies that start the level (and also the road).
After the initial 4-5 enemies die, your partner says "reinforcements arriving" and a car comes, with 3 men inside. As you are facing it, just toss a grenade under it. The men take a while to get out and blamo, your grenade will take em all out!!

This is a really simple achievement. Simply toss a grenade (press RB) towards a group of enemies.
I got this during the second part of the first mission (not the training/tutorial mission).
There are probably better places to get it but since it's such an easy achievement you will probably get it during normal progression through the game. As long as you use grenades, of course.. :)
