The Grey Alien Arm looks like the image in the achievement icon tile. It can be found as early as level 2 and can spawn during a wave if you have all the modifiers on.
This doesn't necessarily warrant a solution, but I wanted to point out that this was the last achievement I unlocked in the game. This is due to the fact that I turned off all the modifiers via the menu to make the game more manageable. Having done this though, this item will never appear.
If you are having the same issue, just make sure to restore the modifiers and play though again on Easy until you come across it.
