The other five trinkets are obtained by defeating a mini-boss enemy called The Shambler.

The Shambler can be encountered by location a Shambler Alter curio piece which randomly spawns in any of the games dungeons. To fight the Shambler you need to use a torch on the altar and the fight will begin immediately.
If you are specifically going hunting for The Shambler I would recommend taking a party with the ability to reach both the front and back ranks with ease, I personally find Man-at-Arms, Bounty Hunter, Occultist & Arbalest to be a great combination for this as all of them can hit The Shambler wherever it sits. An alternative party would be Man-at-Arms, Abomination, Jester/Plague Doctor and an Occultist.
Depending on the dungeon difficulty The Shambler will have 77/116/158HP. During the fight it will summon two minions to fight with it that can deal high amounts of stress damage while The Shambler can cause heavy bleed or blight on your party so the relevant healing items should be brought along.
If you use the first party build just focus on raw damage. Man-at-Arms should refresh his Riposte status as soon as it wears off and your Arbalest should alternate between marking The Shambler and attacking to get the most damage out of your Bounty Hunter while your Occultist heals as needed.
If you take the second party you want to use Man-at-Arms the exact same way and have your Abomination in it's transformed state using either Rake to deal with the minions or hit both a minion and the boss if possible. Plague Doctor/Jester should focus on stacking bleed and blight as much as possible as Shambler has reasonably low resistances to both (85% at the highest level) the Plague Doctor could also try stunning it if you need some breathing room and the Jester can buff your party/heal stress if needed.
Once The Shambler and it's minions are defeated you will be given one of the five possible item trinkets. You can only have one of each so you will never get a duplicate and once all five have been obtained you won't need to fight it any more.
Once all 14 items are in your inventory you will unlock the achievement.
CONFIRMED - Ancestor's Vintage is not required. Purposefully avoided getting this during my playthrough and the achievement popped once I had all 14 original items.
EDIT: The Shambler also has a chance to spawn in any area if you walk around with your torchlight at 0 (why you would though I don't know, I always keep mine at 75+).
You are also guaranteed to get one of the Ancestor items from the Shuffling Horror boss that you face on your first trip into the Darkest Dungeon so if you want you can limit your Shambler fights to four instead of five. - Credit to Moldy Tacos5000

Shamblers also have a very small chance to spawn inside a dungeon hallway when torchlight is at 0, so save those dark/moon items and go shambler hunting!
Complete 9 long champion (Level 5) dungeons --> 9 trinkets
Kill Shambler ( Use torch on a Shambler's Altar) 5 times --> 5 trinkets
Complete 1st darkest dungeon quest (Level 6) --> 1 trinket
Glitched! For me, the Caretaker trophy popped when I collected all trinkets on VITA! Sentimental relics from our forefathers... did NOT pop on VITA, ever.
Trinkets are pieces of equipment you can find in dungeons. Ancestor's trinkets are extremely rare, have a red name (not all reds are ancestor’s) and are dropped in long Champion dungeons.
There are 14 Ancestor’s trinkets,
8 of the Ancestor’s trinkets can be found in any dungeons
1 ( Ancestor's Musket Ball ) can only be found in the Weald region/dungeon.

!If someone in your party dies and you re-enter the game to restart the battle from the very beginning, then instead of the Darkman there will be ordinary mobs with a probability of 99%, 92% and 88% depending on the level of the dungeon, so it’s better not to die .
