When an NPC has a new dialogue, they will have an asterisk over their head. New dialogue appears in their list in white text.
Each dialogue is only available at certain times in the game (generally during a specific quest). For a full checklist of your progress towards these challenges, press start, then choose "Challenges." Choose "General," then scroll down and choose one to see the list of possible dialogues and what act they appear in.

In general, try to remember to check with each of these NPCs every time you start a new quest. Also, you can't get dialogue from a follower unless they are not your active follower.
You won't be able to get Covertous Shen's act 1 dialogue the first time through the game since you don't rescue him until act 2. I suggest simply being diligent with NPC interaction throughout your first playthrough, then going back and using quest select along with the guide to get the dialogue you missed.
UPDATE: Covetous Shen's "Victory" (act 4) dialogue is obtained by talking to him in town immediately after you kill Diablo. This one isn't listed in the guide that I linked.
This is a complete written guide, for those of you who prefer it. Oddly, a lot of other guides do not cover everything in chronological order. This does.
This may not be the most efficient guide as I typically return to town as soon as a conversation is available. Some conversations last across multiple quests whereas some have a very tiny window of opportunity. So, I suggest following this guide fully to ensure you do not miss anything.
Lastly, this can not be earned on your first playthrough. You need to have all the characters unlocked from the beginning (i.e. new game plus) before you can complete this.
The conversations carry across playthroughs, so don't worry if you miss one.

The Fallen Star
Immediately after gaining access to New Tristram, talk to each of your potential followers. Swap one out, if necessary, to ensure you get all three.

VSully has a great guide, this is not a full solution as Sully has everything perfectly down, I just wanted to say my problem with this achievement, I couldnt figure out the first Blacksmith conversation because its a bit confusing as the (NEW TRISTAN) Conversation is second in the guide, but right when you begin the game and talk to the blacksmith it says (NEW TRISTAN) as the 1st one.

You can get this Conversation very easily after you complete the Game by Selecting The "Fallen Star" Quest. DO NOT choose "Begin Quest", choose Rumford at the Gate. You will start in the Middle of the Town, Haedrig will be standing right next to you and the Option will be available.

You must go through all converstaions with all of the main characters (Deckard, Adria, Leah and Tyrael), followers (Templar, Enchantress, Scoundrel) and artisans (Blacksmith and Jeweler). Some of the conversations are very time-specific, being available during certain points of the main story, but thankfully you can always load up an older chapter, or just catch the conversation on your next playthrough.
To find your conversation progress while in-game, hit select challenges, and highlight General. The challenge related to each character is listed on the quest sheet below in parentheses next to the character.
Notes: For the follower chats, you will need to unlock the followers with each character before you can talk to them on that character. Progress carries over between all of your characters, and unlocking the Jeweler and Blacksmith on one character will allow you to find them from the start on another character.
Note that some of the conversations might not update on the in-game challenge list, but will still work for the achievement. Shen's King Leoric conversation did check for me after repeating the conversation a few times, but completing the rest of the list gave me the check mark and the achievement.

Sometimes it seems to you that you have visited the character several times, but there is still no necessary dialogue.
Do not worry. It's all very simple.
The answer is that some dialogues open only after replaying (for example: normal --> hell, etc.).
Act I: Entry into the Order, Sins, Initiation, Lessons.
Act II: In Search of Truth, Dangerous Diary, Facing Fears, Secrets of the Past.
Act III: The Torn Page, Templar Research, Truth, Secrets of the Scriptures.
