How to unlock the 12-Sided Die achievement in Evolve - Definitive Guide
Abe

Val
So clearly you can expect to be doing less for the rest also i would assume.
Headshots: I found Kraken to be the easiest Monster to face when going for headshots. Goliath is also viable, and easier for some characters. Easiest with Abe as the Trapper because his grenades will keep the Monster relatively still.
Reviving: All reviving requirements can be met with survivors in Rescue. This includes Maggie and all three Medics. When going for these as a Medic, make sure that your Trapper isn't Maggie because Daisy will steal revive opportunities, and when going as Maggie make sure the Medic isn't Laz.
Distance: Some masteries have a distance requirement (usually 40-60m). Easiest to achieve on the Defend maps with high ground (e.g. Colonial Power and Water). Use waypoints (click the right stick) to judge distance.
Stage 1 Monster (tier 3 for Griffin, Cabot and Bucket): Set a custom Hunt match on a map that allows the forcefield effect to limit the size of the map. Change the wildlife settings to low so that it takes longer for the Monster to evolve. In Cabot's case, take the reloading perk so that you can use his dust more often.
Markov/Hyde tier 3 Lightning Gun and Flamethrower: This does not have to be on the Monster; target wildlife and minions.
Val/Hank tier 3 Medgun and Shield Generator: This is worded kind of poorly. According to the official forums, you need to heal/prevent 100% of a Hunter's full health. It does not need to be all at once, but the count is reset when the Hunter is incapped. Set up a custom Hunt match on any map that allows the toxic clouds effect. Steer your AI allies into the cloud so that they take damage, and then apply the heal/shield. This strategy also works well for Caira's tier 3 healing grenade challenge (heal multiple teammates).
THIS WILL ONLY HAVE CHARACTERS THAT GET YOU THE ACHIEVEMENT.
If the mastery DOES NOT say MONSTER then you can damage anything, DEFEND is the best for this with the minions.
RESTART, if the MONSTER is about to die then RESTART, saves a good few minutes and all you stats count as they happen.
If you have the RELOAD perks then you should be using that.
Revives can be done on RESCUE though remember if you need them with DAISY, don't have LAZ and visa versa, ALTHOUGH you can hotswap if somebody gets too close and wait till DAISY gets there and hot swap back. If it says 2 revives in one match then as soon as you get the allotted number restart.
As mentioned by others, the KRAKEN is best for headshots, specially on DEFEND.
If its just hit then use the BEHEMOTH (use the the red skin if you have it, it makes it easier to spot).
Again as others have said make sure you use custom match and unless stated put low creatures and whatever map perk suits you. Also favor HUNTERS.
If the MASTERY isn't based on feats in 1 match try and focus on 1 thing, it makes the MONSTER last longer which over time equates to a whole less loading.
If you swap characters you do not seem to get stats for mastery on both, only the character you chose.
___GUIDE___
ASSAULT
MARKOV
Best way to level up MARKOV is solely in defend. This should not be too hard.
HYDE
Best way again is DEFEND. Residual burn will take a bit of time but in DEFEND you get 2 minions every couple of minutes to burn up.
PARNELL
DEFEND, no surprise.
TRAPPER
MAGGIE
RESCUE for anything DAISY related. Tier 3 restart after you get 2 revives.
HARPOON & MACHINE PISTOL - DEFEND.
GRIFFIN (In Process)
SOUND SPIKES/GAUSS - DEFEND - With the sound spikes I focused only on sound spikes, hotswap and take down the SHIELD if the monster is highly damaged. GAUSS self explanatory.
With LEVEL 1 MONSTERS some people say to use maps with FORCE FIELD but I chose BEHEMOTH and CLEAR SKIES. But also spend alot of time on one map and you will learn the layout. If you try and get 2 spots before you trap the monster you can possibly get 5 before you have to restart.
ABE
DEFEND, you can achieve all of ABES easily in DEFEND.
SUPPORT
HANK (in process)
DEFEND or MAPS with TOXIC CLOUDS for shielding damage. TIER 3 SHIELD has to be cumulative, but if the players becomes incapped it restarts. This will be easier online IMO.
LASER CUTTER/ORBITAL - DEFEND
BUCKET
HUNT - UAV just focus on this. TIER 3 you can hopefully TRAP the MONSTER after the second UAV and then get 3 whilst it is trapped, then you can average 5 a restart. Follow the MONSTER so you are kinda close but not close enough to trap, also as soon as you have UAV fly it straight out and be ready to tag the MONSTER as soon as its untagged. Everything else can be done in hunt, specially TIER 3 as when it levels up you can focus on the damage side, but if your struggling with SENTRY/MISSILE the use DEFEND.
CABOT(in process)
DAMAGE AMPLIFIER/RAIL CANNON - Easiest in DEFEND.
DUST TAGGING - TIER 1 change custom settings to have high wildlife. TIER 2 easiest in DEFEND. TIER 3 I used CLEAR SKIES and BEHEMOTH, the BEHEMOTH can get from one side to another quickly so use other MONSTERS if you prefer, BEHEMOTH is just easier to spot.
MEDIC
VAL
DEFEND or maps with TOXIC CLOUDS. TIER 3 HEAL is cumulative, but if target dies then its resets.
Everything else can be done in DEFEND.
LAZ
LAZARUS DEVICE - RESCUE - use hotswap to stop teammates rescuing people. TIER 3 RESTART after 3 saves,
PERSONAL CLOAK - RESCUE - again hotswap to get better stats, TIER 3 - restart after 3 saves.
CAIRA (in progress) Most likely best online.
HEALING GRENADE LAUNCHER -RESCUE /maps with TOXIC CLOUDS. (Hotswap to get better stats)
ACC FIELD - HUNT.
NAPALM - DEFEND or whilst doing ACC FIELD in HUNT.
But GroudonGo, the servers for this game are shut down! How did you boost this achievement with someone?
While you can no longer play online on Evolve's servers, there is still the option to connect with friends using peer-to-peer. Simply select multiplayer from the main menu, decline to attempt to connect to the Evolve servers, and send out an invite to your friend.
If you read nothing else from this guide, read this:
Most challenges are best completed in the Arena game mode. In the settings you'll want to ensure sudden death is switched on. Start the match and select whichever hunter you are trying to complete challenges for, and have your boosting partner select the monster. Your boosting partner should kill all other hunters so it is just you and him. At this point Sudden Death will activate in 30 seconds. While in Sudden Death, hunters deal 400% damage to the monster. This means that any challenge that requires any form of damage to be dealt can be done effectively 4 times quicker. Just make sure to jiggle the sticks every once in a while as the Monster to prevent the AI from taking over and killing your boosting partner. ;)
One final note before we get into things: As others have said before you do not need to touch any of the DLC characters for this achievement, nor the "reskinned" versions of the original 4 Hunters (Wasteland Maggie, Rogue Val, etc.)
With introductions out of the way, what follows is the complete guide to boosting this achievement with 2 players, circa 2021.
Assaults

And with that, you should have your achievement. One final tip is to ensure while you're doing these challenges to always find something to do that will give you XP. Whether its wailing away at the Monster with your Laser Cutter while you wait for your Orbital Strike to recharge as Hank, or whether its killing a bunch of wildlife as the Monster while your partner does their Bucket UAV tags on you, every little bit of XP you can get will lessen the big grind to 40 once you've done anything else. For reference my partner was Level 37 and I was 36 1/2 once we'd wrapped everything else up. We definitely could have been more efficient with our XP while doing these challenges however. Anything else to add? Comment below and I'll add it! Thanks for reading.
In order to achieve Elite status for a Hunters you must complete all Mastery challenges related to their weapons/gadgets such as the Lightning Gun, Assault Rifle, and Arc Mines for Markov (Assault). Once you get all 3 stars on all of their weapons/gadgets you will gain Elite Status on that character. Do this for all Hunters to get the trophy.
The best modes for getting Mastery involving traps/stationary gadgets (Such as Maggie's Harpoon traps, Bucket's Sentry Turrets, etc.) is Defend because the Monster will always come to you, along with its minions, meaning that you can bottleneck the Monster making it more likely to be hit by something placed in its path. While those challenges involving revives are best done in the Rescue game mode, because revives on survivors count.
You should also note that challenge progress is saved as you achieve it, so it is possible to make challenge progress, quit, and then restart. This can be useful for specific challenges such as with Lazarus where you need to revive 3 people per game.
We also advise that for the harder challenges, you make use of the Evolve: Hunters Quest Companion app as this gives you mastery points to use towards certain challenges in the game (see Evolve: Hunters Quest Companion app section in the guide introduction).

New DLC boosting tactic:
Damage masterys against the monster have got a whole lot easier with the addition of the arena mode. With all but 1 hunter remaining the timer will change to "sudden death mode" where damage is increased by 400%. This therefore means that any weapons that require damage to complete can be achieved faster.
This method can be done solo, just by allowing the monster to kill the other bots, then being skilful enough to stay alive and do damage. Or if boosted then one player should be the monster, where the objective is to kill the bots, and then stand there taking the hits from the other player using the required weapon, until its health is low, but not killed. The reason for this is once the monster's health is low the player can run away allowing the monster to regain some health and then go back to being hit, essentially maximising the amount of time the hunter can attack. The best settings for this is a custom arena game with the following extras:
Difficulty: Favors Monster
Wildlife Population: High
Strikes: 0
Battle wins required: 1
Monster starting armor: 100%
Monster starting stage: 3
Also put the time to infinite so that sudden death stays active for the longest amount of time. The number of battle wins can also be changed to its maximum if you want to do this in a longer session with a lot of rounds.
Companion App: (Note this app will be discontinued on 13th April 2016, so therefore boosting mastery progression linked to this will no longer be available)

We set up an arena, any kind, the 3rd level monster Goliath Meteor (he is the fattest of all free ones, if with a friend) or the 2nd level Behemoth (if a bot), the advantage is in the direction of the monster and we begin to swagger.
Markov: We pump up chain damage in the “Defense” mode on 2 carcasses that are picking generators (5k damage per battle). We place the mines for pumping in a row (in a line) and let the monster go at them...surprisingly, the same is true at level 3 (it is not necessary that he attack 2 at once, the main thing is that he does not cause damage while walking to the 2nd mine..and following).
Mega: The main leveling is in the "rescue" mode. The harpoons swing simply, we put 2, a friend runs into them, we put the 3rd...4th... while one rolls back, the next one works and it turns out that EVERY TIME there are 2 harpoons on the monster
Griffin: Ideally, harpoons can be downloaded on the Kraken; in one of its jumps you can place 3-4 harpoons, the monster’s jump cooldown is faster than the end of its action. Sound pegs can be downloaded in the arena; the difference between installation after a knocked down peg is at least 5 seconds.
Abe: Arenas often have 60+ meters of open space. Upgrading your range skills is a pleasure (click , on the stick to find out the distance to the carcass)
Val: Don't even think about leveling up on bots with this character, ask a friend to play as a support or a stormtrooper. The monster will still attack ONLY the MEDIC. With the tranquilizer, just like with Abe.
Kaira: The main place for leveling up to lvl 3 is the “Rescue” mode, suffocated ones are considered as normal players.
Lazarus: Also the rescue mode (the resurrection of fallen dead goes to “+” as for normal people). It’s not trivial to pump up 2 skills for lvl 3 in a few rounds.
Hank: Let your friend play as a medic and you will be a pear, which the monster will always try to get away from, on which we pump up shields. In the case of orbital bombardment, everything is like with Abe.
Bucket: In general, the same feature as with Markov and Meg - we install 3 turrets, shoot a little, install another one (the old one dies) and so on until the skill is leveled up. It’s better to detect a monster in the arena; it’s 4 times faster.
Cabot: Just the arena again. At the 3rd stage, "Damage Booster" is 50% of the normal charge of the magazine, so 50% is gone, you release it and use it again. It turns out by X2 multiplier.
Also in this application you can get 12 foreground icons by leveling up your characters to lvl 30 and 4 more for opening all the monsters in one of the biomes.
