This achievement is obtained by completing the main storyline following the quests of one of these factions: Minutemen, Railroad or Brotherhood. You only need to do one of these, not all three.
While the other guide is helpful, it is rather chaotic, so I made a chart that shows how to obtain all the achievements with only one save / reload. My guide shows how to unlock "The Nuclear Option" while aligned with the Brotherhood, but I also indicate when to diverge from these steps, if you want to play the Railroad or Minutemen endings.
Scroll down for a more detailed explanation (inside the Spoiler Tag, as it is an awful lot of text).
For starters, keep in mind a thing: having high charisma will help you when you'll have to balance several factions at once. I recommend having at least 6-7 points of charisma, and using "charisma-enhancer" gear like Glasses, Hats or Suits, or even using, when neccessary, beer or some drugs as Grape Mentats or Day Tripper that give charisma points for a few minutes.

START MAIN MISSIONS
As soon as you're out in the Commonwealth, you'll be able to explore freely. Follow the main quest for a while, and, after completing the quest "Reunions" the Brotherhood of Steel will be available. That moment, or a little later (after "Shattered", for example), is a good time to join the available factions and advance on their questlines. I recommend advancing until your current mission is "The Glowing Sea", that way you'll have a higher level to face these missions.
Do three of these secondary missions, and the next mission, "Taking Independence", will be available. Do it, and then the next one, "Old guns". Upon completing these, you'll have all Minutemen achievements, and you can forget of this faction for the rest of the game if you wish.

Time to join the Brotherhood. You'll find a military radio channel that will lead you into a police station, where you find some Brotherhood members. You'll have to complete a few missions until you become a member. "Reveille", "Fire Support", "Call To Arms", and, finally, "Semper Invicta".
You can do a few more quests for them, and I reccommend to do it. These are available after you complete the main mission "Reunions", and are "Shadow Of Steel", "Tour Of Duty" and "Show No Mercy".
Railroad time. You'll only be able to do a couple of missions for them at this point. These are "Road to Freedom" and "Tradecraft", in which you join the faction.
Time to deviate from the main quests and work in the factions again. As soon as you complete "Institutionalized", start "Underground Undercover" for the Railroad, and advance in the quest until your objective is
continue working for Father.
Then, switch to Brotherhood missions. Complete "From Within", "Outside The Wire" and "Liberty Reprimed". Now "Blind Betrayal" will be available, but DON'T COMPLETE IT. If you do, Railroad quests will turn unavailable. So, let's get back to the Institute.
REACH THE DIVISION POINT
Complete "Synth Retention". Then, in "The Battle Of Bunker Hill", you'll have two optional objetives. Choose to
warn the Railroad, and do what they want you to do.
This is the DIVISION POINT where the different paths to the ending start. I believe this is a good moment to get all miscellaneous achievements done (Bobbleheads, Miscellaneous Quests, Secondary Quests, Giant Enemies, Settlement Hapiness, etc) so you can then pick tne ending you want and conserve all of the benefits of these tasks.
And here's the important point. As soon as the next mission, "Mass Fusion" asks you to make a choice, MAKE A MANUAL SAVE. I'd create 3 or 4 saves in that moment, for precaution. Write the location and time of the save in a piece of paper so you can recognise it easily later on.
And now, you'll have 2 different paths to the ending.
PATH 1: RAILROAD + INSTITUTE
In "Mass Fusion", choose
go to Mass Fusion.
This will turn you into a permanent enemy of the Brotherhood, making their quests dissapear. But you can still work for the Institute and Railroad.
Complete "Pinned" and "Powering up" for the Institute.
Continue with the railroad questline. Dependind of the secondary missions you have done for them, you'll have to do a quest named "Operation Ticonderoga", or you may jump directly to the next mission, "Precipice of War". Anyways, complete those and the next one, "Rockets' Red Glare", for the last Railroad achievement.
Stop here, because the next Railroad mission ends the main questline. Time to go back to the Institute and complete their missions (You can make another save here and go back later for the Railroad ending, if you wish). "End Of The Line" will turn the Railroad hostile, and, because you already did "Rocket's Red Glare", you'll skip "Airship Down" and jump directly to the final Institute mission, "Nuclear Family".
This will turn you into a permanent enemy of the Institute, making their quests dissapear. You'll be able to work for the Railroad at this point, but we won't do that.
As soon as you finish "Mass Fusion", "Spoils Of War" will be available. Complete it (make a save here if you want to do the Minutemen ending later), and go on the next quest, "Blind Betrayal". Now we can complete it without messing the other achievements.
Why do I say that? Because, as soon as you complete this quest, the next, "Tactical thinking" will start automatically... and it will block the Railroad quests, making them unobtainable. But at this point tat doesn't matter, because we will obtain them in the other path.
After "Tactical Thinking", complete "Ad Victoriam"
Railroad and Minutemen have their own "Nuclear Option" too, but only one of those is required for the achievement. So if you don't want to end the game with the Brotherhood, reload one of the two optional saves I mentioned, and complete the game with either of those factions to get the achievement.

Which is the best fraction to end with so that I have the most fraction related and secondary quests to do after any of the Nuclear Options? E.g. when I choose the Brotherhood, can I still do the Minutemen farm recovery missions or will they be unavailable?

When you say manual save do you mean quicksave? Or is there a way to copy our saves? Do our quicksaves disappear?

No, I mean using the "Save game" in the pause menu, not the "Quick save", because quicksaves are overwritten periodically. Regular saves aren't.

If I wanted to do the Railroad ending first, for like my main save, and then go back to the Optional Save #1 and do the Institute ending with that save? Would that be ok or would it not work? Thanks.

Yes, it is completely fine. You receive the nuclear option with every faction ending besides the Institute. Do if you do that and then follow the rest of the guide you'll get all the achievements equally.

Okay, I think I know what happened. I was playing on the hardest difficulty: Survivalist, using stealth and a knife as my primary weapon. I also have the melee perk maxed out which means that it does double damage to everyone in front of you. I normally play alone and so I suspect that due to the manic nature of battle I must have hit Z1-14 enough to make him go hostile. He's obviously not very forgiving! I reloaded a save at the start of the mission (fortunately I'd save just before you teleport into the control room to talk with Z1-14) and kept checking before I did any more saves that he was still "green" in VATS. It seems that when you get to the very end and after the mission completes and awards you XP you get another boost of XP for some reason. It was after this second boost that the achievement popped. Also quite interest is that when it did glitch you couldn't' even jump off the top of the building. It was like an invisible barrier. When Z1 wasn't hostile the invisible barrier wasn't there and you can jump to your death, if you felt so inclined.

Great, thank you. But I'm not sure why it says that you cannot do Blind Betrayal because I did it forever ago and I still can do Railroad missions. Only after Tactical Thinking do they turn hostile.

Hott Cosby, yes, it is. Siding with the railroad makes the process a little easier, but you can follow the guide if you build the device with other faction. Just... when you visit the railroad and Desdemona confronts you about not building the device with them, be careful with the dialog options.

Nooo - got a gamebreaking bug following the BoS ending. Avoiding spoilers, it's the bug where you get teleported to your next objective with this really bright light, but before the light dies away, you get teleported away. Can't complete the final quest. >:-(

I built the transporter with the Minutemen & went through the main quest line through Synth Retention & still got Underground Undercover when I went to visit the Railroad. Thought I was screwed out of that quest when I saw this. Phew!
Edit: If you did the same as I, when you talk to Desdemona to obtain Underground Undercover, you will have to warn her for the optional Battle Of Bunker Hill objective (as that quest will automatically be given to you upon completing Synth Retention) before you can talk to her to receive Underground Undercover. Simply warn her for the Battle Of Bunker Hill quest then ignore it & proceed to talk to her again to get Underground Undercover & you can follow the rest of the graphic as normal.

I had to do a whole second run through for the Railroad and the Brotherhood Acheivements... accidentally saved over my pre-Mass Fusion save... so I downloaded the flow chart and marked off all of the acheivements that I already had in Paint :) Thank you

Your welcome! I don't remember exactly, @UltraAmbiguous, but it is possible. Anyways, given that it is a main mission, if there's an advanced terminal, there should be a way of finding the passcode around in case your character hasn't hacking skills.

Only a thought... Why can't you complete Blind Betrayal straight after Liberty Primed, especially if you elect to save Danse through dialogue options? Isn't it the next mission Tactical Thinking that the Railroad get annoyed? I'm only asking as I want to get Danses perk, before the split save, then I can get X6-88 perk the final play through and have all perks. Your thoughts and advice would be welcome.

That would be the Minutemen ending. If you reach it from the point I marked as "Optional Save #2" in the guide, and as long as you don't complete missions as the Brotherhood's "Tactical Thinking", Railroad's "Rockets' Red Glare" or Institute's "End Of The Line", you'll end the main quest allied with all the factions except for the Institute. Some type of secondary quests from these factions will not be available anymore, but you'll still be able to work for P.A.M. on the Railroad, for example.
Actually, I believe there's no way to turn the Minutemen hostile (except for shooting them in the face for no reason), so if you complete the game with the Institute, Brotherhood or Railroad you'll still have the Minutemen missions available.

For some reason this achievement glitched for me on the Railroad route. Somewhere towards the assault on the reactor Z1-14 turned hostile and every time I spawn in a new area he starts attacking me. I have to keep taking him down, but you cannot kill him; he just gets back up after a short time and starts attacking me again. When I get to the end I blow up the Institute, talk to Desdemona, get the XP for the mission, and get told it is complete, but... no achievement. Plus I cannot leave the top of the mass fusion building; fast travel is not available, probably because Z1 is considered an enemy and he's too close. I did a search and I've found others talking about this glitch. I suspect that I'm going to either have to go back to an earlier save and redo the entire mission again or get the achievement on my BoS completion. I absolutely loved this game and this is the first bug in the entire game I have found. I was level 66 when I completed the main story line and I've still got more exploring and side missions to do!

@alandonovan Looks like it isn't a common thing, and maybe it has been patched already, but it happened to me and a few others: as soon as I completed Blind Betrayal, Tactical Thinking started automatically. When I went to the Railroad with this quest active, I couldn't interact with any character, so all Railroad quests were blocked.
Some people didn't have any trouble, so maybe this was a glitch on my playthrough. I just didn't want to guide anyone into an error, so I preferred to write the quest guide that way, just to be sure nobody runs with that same problem.
So, if you want to complete the mission after Liberty Reprimed, try to see if it works fine for you and you are able to get all the perks, but make a save before ending Blind Betrayal so you can go back in case this problem happens.
*Edit 1* So during Mass Fusion for Institute before you use the relay save as this quest makes the BoS hate you. So... Up till this quest you're good to do everything for all three factions. If you are about to make a faction hate you, you will get a notification. cancel and save before.
And you're good up until End Of the line for the Institution for the Railroad
You can get this from Minutemen, Railroad, and BoS. Don't worry about Minutemen
Brotherhood of Steel - Talk to the old man, since you sided with BoS you will already be at C.I.T. Pack some explosions and energy weapons and fight the Synths. Go through the hatch, keep going to the core. Take the elevator to get a password from Father. Use the Terminal and sound the alarm to avoid more pointless fighting. Keep running to the core. Talk to elder to leave but Ingrum will talk to you about saving a Synth or not to save the Synth. You teleport to the roof and watch the explosion.
Railroad - Listen to Desdemona for the plan. Teleport and you see some tin on tin action.... (not like that). You're friends port in. Drop through hatch keep fighting (explosives and energy weapons). Run to the core, go to the elevator get the password from Father, use the terminal (you can engage the alarm if you want but it's not needed). Go to the core, fight some more. Use the door activate the core. You get ported again, save or don't save the Synth. Go to the Relay then you're outside. Talk to Desdemona and set the explosion off and Achievement..
Read my guide for this one if you have made the saves I've been talking about.

I am doing this quest on behalf of the minutemen and even with this guide I cannot find where I am suppose to jump in the water and find this tunnel. I believe the raider camp you are referring to is the Wreck of the USS Riptide? If so I still dont see a tunnel anywhere. I have been searching for hours and there is no videos or pictures anywhere online showing where this tunnel is. if you could help me a little more id appreciate it.

Quest Givers: Elder Maxson (Brotherhood), Preston (Minutemen) or Desdomona (Railroad)
Location: The Prydwen (Brotherhood), The Castle (Minutemen) or Railroad HQ (Railroad)
The Brotherhood, the Minutemen and the Railroad all lead to this achievement. This is essentially the end game quest for these three factions. While they all have different routes to get to this point, the end goal is the same (spoiler free) and it will unlock prior to "Prepared for the Future".

This trophy can be achieved by finishing the final quest for either The Railroad, Brotherhood of Steel or the Minutemen. Each Faction has a very similar ending to each other, which is why this trophy is universal to all three factions.
