How to unlock the That's Just Crazy achievement in Gears of War 3 - Definitive Guide
To unlock the Insane difficulty, you must complete the campaign once on any difficulty. I would play it on Normal or Hardcore just to be better prepared for Insane.
YOU WILL NOT BE DOWNED ON INSANE. IT WILL BE INSTANT DEATH WHEN YOUR HEALTH DROPS TO ZERO.
General tips:
-Play Smart.
-Stay behind cover during combat whenever possible.
-Know your enemy. Know what they are able to do.
-Try to kill the lesser enemies first. It will make it easier in the long run in most areas. This is due to much of the crossfire. So kill the lesser enemies to eliminate the problem of getting overrun.
-There have been numerous reports that if you give a power weapon (boom shot for example) to an ally AI, then they have infinite ammo and use the gun with deadly precision. So don't be afraid to let them have most of the fun ;)
Note: It is easier to play it with a friend, as you could play it on Insane and they could play it on Casual. This will allow them to mow through your enemies with ease. This also would work with a 2nd controller, if you keep the Insane profile back during combat. All you have to do is play carefully.
Additional Note:
If you play this in arcade, then you can use mutators. These change game experience depending on the mutator. There are easy mutators, which make it easier to complete, but will give you less experience towards leveling up while active. There are hard mutators, which will make it harder (of course) but give you more xp toward leveling. And then there are fun/cosmetic mutators which have no effect in difficulty or points.

Since this can be unlocked in arcade, I'm assuming you can have any of the modifiers turned on. Is that right?

Correct. Thank you for reminding me of the mutators (modifiers), as I neglected to remember them at the time of writing. You should be able to obtain the achievement with mutators in arcade (if you can unlock them) unless for some reason it's changed in the future. I'll update the solution and give you a bit of credit.

This is a full singleplayer guide to the campaign on it's hardest setting with full commentary. For the most it's pretty standard, but some sections are bat shit crazy.

Started to watch your videos again but managed to pull ahead and complete the campaign before you had them all uploaded. Either way, fellow TA members... Seraphim delivers with his walkthroughs!

That’s Just Crazy
Really, Insane difficulty isn't too bad save for four spots that took me an hour, or more, each in frustration: the Lambent Zerker (Rescue), atop the gas station (Brothers to the End), any part with an Armored Kantus (Bon Voyage and later on), and lastly the Tempest (Reckoning).
I beat this solo in regular campaign mode, never did I use Arcade multipliers or a second controller. The following is less a specific strategy and more a collection of useful tips that I’ve been told or figured out during my run through.
General Advice
Big Guns Go to A.I: As stated by other players and confirmed by myself, giving your friendly A.I. explosive weaponry is a plus (e.g. Torque Bow, Boomshot, even the Longshot is good). They're better shots than you'll ever be and they don't run out of ammo.
Distance & Cover: This game was designed with the cover system in mind. Most of my time on Insane difficulty I spent crouched behind cover, well behind my A.I. teammates, taking potshots at the Grubs n' Glowies. Also, there were some parts where I would activate the enemy spawn then turn tail and run to a previous section--not anywhere near where EPIC wanted me to be--simply to avoid all of that crossfire that'll be coming down on that spot. Thus, leaving my A.I. buddies to fight for me while I snipe from a distance.
Specific Advice
Lambent Zerker (Act 3, Chapter 2): Not the most difficult boss to beat but, she still took a while to put down. Obviously, shooting the heart is the only way to hurt the Zerker. Shooting the Zerker before her chest cavity is open is not advisable. This will cause her to charge toward you before her chest is open.
After a little while, she’ll start to bleed imulsion, leaving trails of it that’ll kill you instantly. After dodging an attack stay as close to the trail as possible, making it less likely for the Zerker to box you in with the imulsion trail, provided you’re A.I. teammates don’t screw you over. After she has made a few passes at you and your available safe standing room has shrunk down, remember that there is that big generator/building/pillar thing and you’ll be safe if you can get that between you and the Zerker. This will give you time to reload, grab ammunition, revive teammates and let the imulsion trails dry up.
Aside from the above, just keep dodging her attacks and whittling away at her health and you’ll beat her easily enough.
Gas Station Rooftop (Act 3, Chapter 5): The beginning of this fight isn't the tough part. Simply kill some Boomers without getting hit. I found the left side of the rooftop to be preferable to the right; perhaps due to less splash damage from the extra cover on the right. After the Boomers are all down the Grapplers will begin to attack you, kill the first wave of them.
There are only a few Grapplers so you must be quick with the next piece of advice. After the first wave of Grapplers is down, run to the rear of the rooftop and plant an incendiary grenade. Feel free to plant a regular grenade along the Formers’ path far enough away to not be activated by the incendiary one. Don't plant them before all of the Boomers are dead, I found that their stray shots almost always blow up the incendiary grenade, messing up the battle strategy.
Now, finish off the second wave of Grapplers, if your A.I. buddies haven't already, and then the Formers will show up. Your first grenade will take out many of the Formers and slow them down giving you breathing room to deal with the continuing assault of Grapplers. I only needed to survive, dealing with the Grapplers, Formers and the Glowies, for about 1 or 2 minutes before the cut scene came to the rescue.
My advice for those 2 minutes of hell: don't take cover, always be on the move, hop over any wall you accidently lean on and use the chainsaw on the Grapplers; while in the chainsaw cinematic you can't be hurt and your A.I. friends may just kill any other baddies that are around you.
Armored Kantus (Act 4, Chapter 5): When I first encountered these guys on Insane I spent nearly 3 hours of dying, restarting and dying again. This was before I realized the power of giving the A.I. the power weapons. There’s ammunition on the left side of the dry dock and Boomshot on the right side. After you’ve killed the first Kantus--I ran around him and stuck a grenade on his ass--there should be one more in the background with a Theron Guard and a Drone as backup. Feel free to kill those three guys in whatever order you like but, try to get to the far side of the dry dock (i.e. opposite of where you entered) before you kill the last of them. This side of the dry dock has better cover and your being there before the third guy is down will, more often than not, make the reinforcements appear on the other side of the dry dock (i.e. near where you entered).
Enter the Seripedes and cause for many a reloaded checkpoint. First, I suggest you take down as many of the easier Drones and Guards as you can, to cut down on the crossfire. After they’re down take down the Kantuses while making sure the Seripedes don’t charge you. If the Seripedes charge anyways, and they probably will, run, don’t take cover, don’t try to circle them, just run.
I finally made it past this part while running from a Seripede, rolling toward a Kantus, sticking him with a grenade and then teaming up on the last Seripede with the rest of my buddies. It was simply luck that kept the Kantus from shooting me as I was rolling.
You’ll be meeting more Kantues throughout the remainder of the campaign but, they won’t be anywhere near as difficult to kill as they are in Bon Voyage, thanks to the absence of the Seripedes and the inclusion of better cover.
Some general tips for killing the Kantuses- if you can find enough cover your A.I. teammates will eventually (read: 5 minutes) kill an Armored Kantus using regular weaponry (e.g. Lancers and Hammerbursts), giving your A.I. the Boomshot or Torque Bow makes this considerably easier, a Kantus can be knocked down by a nearby explosion or a direct Torque Bow hit and when they get up they’ll spend the next second or two screaming at you (i.e. enough time to send another Torque Bow shot into their face or plant a grenade on their back), and, lastly, it takes 4 thrown grenades to kill a Kantus but only one stuck to their knickers
The Tempest (Act 5, Chapter 6): If you attempt this final boss without a strategy, good luck and have fun being stuck there for the next few hours. The first part of the fight is fairly straight forward; take cover when the Tempest does the dragon-Sun-scream-thingy and shoot it in the mouth when it’s not screaming.
After the Tempest starts to physically attack Adam Fenix’s lab is where you’ll need to really be careful. You’ll have Guards and those floating machine guns shooting you, a Tempest to kill before she kills Adam and near useless cover. The best place to be is at the elevator you took to get up to the lab (credits go to KingBroly).
It is more difficult to shoot the Tempest from here but, you’ll be further away from the secondary baddies that are just plain annoying. After you shoot the Tempest off of the lab the first time, it’ll start flying around breathing fire and laying down the hurt. Just keep circling around the elevator for cover and, most importantly, keep shooting the Tempest in the face.
As soon as the Tempest is injured enough and resting on the ground whip out the Hammer of Dawn and aim it at the Tempest’s face. TO KILL THE TEMPEST WILL TAKE TWO SUSTAINED FACE SHOTS with the Hammer versus the more than three non-face shots. To get the shot you’ll need to get out of the elevator cover but, when the Tempest is on the ground you won’t be harassed as much as when it’s on the lab. After the first shot to the face the Tempest will take to the air again, run back to the elevator and do all of the above one more time and you’ll kick its ass in no time.
Some words of advice: don’t shoot the little baddies (e.g. Guards and the gas-bags), the most dangerous part in the fight is the search for ammunition so if you’re being harassed by one of the little baddies simply “spot” him for you A.I. teammates to kill.
Being this far away from the Tempest means you’ll be pissing through more ammunition and taking longer to shoot it off of the lab. At this rate the lab will only be able to survive two attacks so, if you mess up, you’re dad will be the one paying the price.
Last Words
Again, the above is simply a list of suggestions not a full guide. The majority of the campaign is fairly simple to move through without too much restarting. I beat the campaign on solo without any arcade mutators. What I have written above is the product of a great amount of trial and error by myself and a bit of help from KingBroly. I wish luck to anyone else going after this achievement.

For the final phase of the Final Boss, you can hide behind the Elevator that you came up in at the start of the Chapter. Therons will very rarely charge you (1-3% chance this happens), and will provide ample cover from Shriekers. You'll have to aim through some trees but that's okay since you can still hear if you're hitting the Tempest's mouth.
With this strategy, you'll be able to focus on the Queen until she's down, then you can rush for a Hammer if you need it. If she's down where the Hammers are, stay there for quick use.
EDIT: It works Solo and in 2-player Co-Op so far. I don't know about 4-player Co-Op.

Little tip for defeating the final boss on insane. Start up a match with a partner at the very end of the mission. Have the person getting the achievement set the difficulty to insane and your partner to casual.
If your difficulty is on insane: Sit back and watch your friend kill the beast in less than 5 minutes.
If your difficulty is on casual: Kill the beast.

This achievement has turned out much easier than I ever expected. To make things as easy as possible you'll want to play through in arcade mode. As others have said this allows respawns as long as you are playing in co-op and there is at least one player still alive. This factor makes it a good idea to work on the 4 player co-op achievement along with this one a good idea so you'll have more people to help keep the session moving along.
Another key to an "easier" experience is using mutators. I've found that super reload works wonders if you have a knack for perfect active reloads. With an effective super reload you can cut through nearly any enemy like a hot knife through butter. It actually seems a little unfair at times, but I'm not complaining. Unlimited ammo will help also, but it's really not necessary if you have super reload and are comfortable using a variety of weapons. Super reload not only beefs up your weapon's effective damage, it also increases your ammo with perfect active reloads.
There's really no magical method to finishing the campaign on insane. You still have to use cover alot as insane level enemies will tear through you with only a few rounds. Blind fire works wonders, like crazy with super reload. It would also be wise to have one person hang back so that there is always at least one person in a safe spot to allow others to respawn.

- Infinite Ammo Mutator if you have it
Another key to an "easier" experience is using mutators. I've found that super reload works wonders if you have a knack for perfect active reloads. With an effective super reload you can cut through nearly any enemy like a hot knife through butter. It actually seems a little unfair at times, but I'm not complaining. Unlimited ammo will help also, but it's really not necessary if you have super reload and are comfortable using a variety of weapons. Super reload not only beefs up your weapon's effective damage, it also increases your ammo with perfect active reloads.
There's really no magical method to finishing the campaign on insane. You still have to use cover alot as insane level enemies will tear through you with only a few rounds. Blind fire works wonders, like crazy with super reload. It would also be wise to have one person hang back so that there is always at least one person in a safe spot to allow others to respawn.

This is a solution to finish the game (beat the last boss) on Insane with 2 or more players.
There is a little trick that really helps out. Go all the way the acts on insane (follow the tips on the other guides, which are very helpful and complete by the way) until the final boss. There, beat him until you reach the checkpoint. This first part should not be a problem even if you are on your on since you only need to worry about the boss itself. Take cover everytime he start shooting his beam and shot his mouth until he falls, then use the Hammer of Dawn.
When you reach the checkpoint leave to the main menu. Now one person should start the last chapter on Insane playing solo. This person needs to defeat the queen by himself. After the checkpoint, go take cover at the elevator, there you can protect yourself from the beams and will have a large window to shot Myrrah. You will have to pick Hammers very often (sometimes I had to use 2 and a half charges on the boss before he starts flying again) but this should not be a problem since the number of guards is almost zero and the bots will kill anyone who eventually comes up. When you defeat Myrrah, as soon as the cutscene starts, pause the game and invite your other friends. Once them all are connected, unpause and... achievement popped, for you AND your friends. The trick is, this chapter have 2 checkpoints, the start and the checkpoint when Myrrah starts hitting the tower. Since you all played the first checkpoint and joined before the second checkpoint ends, the game counts as you played the whole chapter.

When you are playing on insane with friends, after the checkpoint the Palace Guards will start tho show up. With 2 players they seem to appear endlessly which makes things VERY difficult. With 4 players it's just hell. Using this method you will barely see a Palace Guard and will kill Myrrah in no time.
Questions related to this method
- After the checkpoint I had to hit Myrrah twice with the Hammer and then the cutscene triggered. If you want to be more cautious you can invite your friends right before you shot her with the Hammer. In this case I can't confirm if that will be the last shot or if you'll need more (since the number of players increased)
- The players who will join the game later MUST all play on insane until the chapter's checkpoint. This is the only way they gain credit for completing the chapter in the end (They can't just join you in the end and expect getting the achievement)
- Quitting to the main menu is required because if all players but one leaves the match, the Guards will continue to come up as if all the players still there (I mean in quantity).

This guide is just for the final boss.

IMO your weapons should typically be the lancer, the torque bow and the boltok pistol. You need to spend a lot of time in cover on insane, and this makes both shotguns pretty worthless, but head shots with the Boltok valuable. The last time I used the shotgun was on 'brothers to the end', picking up the torque bow as soon as I could on chapter IV, and these weapons comprised my load out for the rest of the game. You should still use it sparingly, but there are plenty of enemies that drop torque bows in this stage of the game.
Most of the enemies can be killed quite quickly once you know how to kill them. This is one reason for keeping a torque bow handy, it's only two shots to kill a armoured Kantus, the first shot stuns the Kantus and makes him scream, shoot for the red slot in his head and bingo! 2 shot dead. Gunker - you will die sometimes doing this, but a grenade tag usually kills a Gunker immediately. Drudges can be killed quite quickly with a torque bow head shot, once it has mutated into the flappy arms state. Also, you are nearly always right to shoot the pods on a stalk, it's a priority nearly every time.
Brumak section is quite easy, just need to roll away when it fires missles when you are in cover in the last container. Shoot the missle launcher on its head, and if it turns away shoot the fuel pack on the back, that's the weakest spot.
Lambent Zerker - You can actually grenade tag the zerker when it is in its scream phase, if you get the timing right it will open its chest just before the grenade explodes, but you're still going to have to do a stack of shooting anyway, that's just for the fun of it. Generally though the trick is to try and make the immulsion stripes parallel to each other so that the least amount of space is taken up, and if things get a bit tight run around the pillar and let the immulsion dry away.
Tench Run - when trying to hijack a barge. Make you sure get a torque bow as soon as possible and keep opposite ends from the barge hiding behind cover. When they drop the rope in pairs already have the torque firing up so you can just release when ready. As you run around a lot in a circle you'll eventually get to pick up dropped weapons, running in and getting them will be your big undoing.
Brothers to the End - you don't need to shoot at anything coming out of the tunnel, take cover behind the gas cylinder and wait. After a while you will get grapplers, shoot the hooks and also spot ones that do make it up and hide again. When the remnant humans start attacking, take note of where they come from. Keep shooting at them with the lancer and don't worry about anything else. The lambent arrive, but keep shooting the remnants and once they are done sprint to the end that they emerged from, you should be safe.
Bon voayge, the dock fight - as you've got a torque bow this should be relatively straight forward once you've got the rhythm of things. Take on the armoured Kantus first from cover, and there is a Theron guard with a torque bow which it will drop if you run low on ammo. Ignore the serapede, well, you got to move around but don't shoot it, not yet. Get the 4th and final armoured Kantus out of the way, kill everything else and then work on the two serapedes - remember to keep spotting it, ideally you want to get behind it from a reasonable distance away to get a good number of shots in.
The tempest in Azura - don't waste your ammo shooting at it, just keep heading up the stairs avoiding the solar weapon and eventually you get to chainsaw the restraining cables. Simples.
The tempest itself, well plenty of people have talked about it, first part is easy, just keep in cover and time things right. Second part, very tough, when you kill the queen's gaurds replacements come grappling up the wall so it's not worth getting involved if you can help it. I did this (eventually!) by being fast at it, this prevented the normal losing showdown of having to taken on lots of guards during the second time of shooting the tempest from the tower. I didn't particulary find any of the scenery a safe place to hide behind.

I have another quick tip for the final boss on insane. Have a friend play solo and jump into their game after he starts to play. The game seems to scale the difficulty to the amount of people who start the chapter together. Be warned that we found that if you die and restart the checkpoint, the difficulty scales to however many people are in the game, so you should quit out and have your friend start the chapter over.
finaly got this cheev after 3months of trying. the game itself was no problem, its just the final battle. its nearly had me in tears a few times not to mention the ammount of rows me and the misses have had over the time ive spent on it.
completed it in arcade private match on my own and nothing happened no cheev. gutted.
so did again on regular mode, (not arcade) with fakkie twistzz he was on casual i was on insane with unlimated ammo, super melee, and super reload. and it still took about 10 goes. when you get to the 3rd phase of the battle hide behind the elevator shaft you came up in. its harder to shoot things but gives you more cover
i found the more people you have with you the harder it gets, so i would recommend that you do it with two people max.
when shes on the tower both must shoot her down, then casual player takes out the gaurds and screechers, thats the best advice i can give, hope this helps

This the way I did it. Start it up on standard. This is the reason why: you won't die in one hit unless it is the kill light and the checkpoint saves, so you can go back to it anytime you like. There is a glitch where the palace guards don't spawn. When this happens, this is the best way to beat the game, but it is a glitch so it doesn't happen all of the time. I don't know what causes it. Load your checkpoint and try to beat it. You will notice the glitch when your allies start shooting the boss at the second part of the level.

If you have another controller you can just leave it on and get the co-op 50 gamerscore achievement, or you could actually have someone playing with you. Eitherone, you still receive 100 gamerscore plus collectables plus any story related achievements in your first time!!!

First thing first: You don't have to complete it during the campaign. You can also do it by completing the chapters in Arcade mode: Which is good, because you can use mutators, such as Infinite Ammo and Super Reload, which can double the damage of your guns. Also, if you play Arcade, you don't have to complete the game in order. You can switch and play any chapter you want, completing the easier ones first and let the hard ones later. If you feel lucky, you can also search for Insane co-op games, maybe there are some guys who are hell of a good players, or invite some of your friends who are familiar with the game. Using a second controller and setting it Casual (while your main profile is in Insane) could be another solution, feel free to try it, but i did not prefer this method. (and neither tried it)
During my walkthrough i used mutators. (because why not) The Lancer is very powerful, accurate and easy to reload. The Retro Lancer has better damage value, but it less accurate and you need 2x-3x more time to reload it than the regular Lancer, so i don't recommend it. Also, if you find powerful weapons (for example a Boomshot) give it to an NPC (left trigger + X) because they have infinite ammo and as i noticed, they never get killed, even if they are down.
Let me give you some tips for some of the maps:
Leviathan Boss:
There is a very good video guide about it, but i'm gonna write down the very same:
First, get in the Vulcan and when the beast is coming up, shoot out the right eye (the bigger one) If it pops out, some polips gonna come out from it. There is a container on the left of the boat. Go behind it, and shoot the Leviathan from there. When the second time it comes back, shoot out the left eye too (from behind the container, because there you have a very good look to it) When it pops out too, just aim to the mouth. Some polips and Lambents gonne jump onto the ship. Feel free to kill them with the Vulcan, or just jump out from it and use your guns. (I recommend you the second, because then they cannot damage the Vulcan, but you need to be extra carefully!!!) The last time when the beast come back, press A and use the rockets. Aim to the mouth and bamm, the freak is down.
Helping Hand:
At the end of the level, you got ambushed some big f.ing monsters. First, it gonna come from the right side. You can easily deal with it. When the second one comes out, there are the problems starting, because its throwing fireballs to you, and you got killed by one hit. What you need to do is to run behind a good cover (for example behind the barrels on the right, it's a very good spot) and wait until your team is deal with it. Easy and simple. You can shoot the pods during the fight if you want, but it's really not necessary.
Corpser Mother:
It's easier than you think. Just deal with the baby corpsers, because they can do very big damage, and when the place is clear, aim to the eye of the big ugly mf. When she moves back, some tougher corpsers gonna come, maybe 2-3 in a time. All you have to do is to running around in huge circles and never, NEVER STOP!!! You can see the kill messages on the bottom of the left, and after your teammates killed all of them, you can focus to the mother again. When she get only 1 eye left, you will have a checkpoint, so it's relally not a big deal.
Treshold:
In the part, while Jack is working on the door and you must fight against hordes of Lambents, i discovered that the safest position is not in front of the door but at the seperated "podium" area just opposite the sandbag cover. Go back there and shoot the pods. The enemies will mostly focus to your teammates, and you can easily kill them from safe.
The final boss is a bit easier if you already unlocked the Comet mutator, because while using it, you are invulnerable. So you can heal a bit and as i know, the Queen cannot damage you with her special attack. Just do the same as you would on Casual and be careful with the other enemies. Or jump in a coop game and hope you can do it with your mates. I did it in coop.

Just thought I should add a nifty little trick for doing this campaign on insane in arcade. If you use the mutator called Vampire, it actually let's you play the game a little more carelessly. As long as you are shooting an enemy, your health will never go down. This is extremely useful during the final boss while she is on the tower. Because you are shooting her in the back, you take no damage, even if being shot. Another mutator I suggest is Comet, since while you are running, you are cannot be damaged. I've been running through the campaign on insane solo with these mutators, as well as super reload. I'm actually already at the final boss, just have to finish beating her. Hope this helps anyone.

the easy insane trick from gears 2 is back in this. what you need is 2 controllers for split screen. select standard campaign (as arcade cannot let the other guest choose their own difficulty) and your main account on ur 1st controller will be on insane while your guest on the other will be on casual. on the majority of the acts u pretty much just have to have your main take cover on the far back to keep away from peril while u use ur 2nd to do most of the dirty work and your main will always teleport everytime you reach a checkpoint. the only parts where u need to move ur 1st controller is when you need to open those 2 handle garage doors and barbed wired fences together to proceed to the next area. the only chapter u should only do on insane solo is the underwater sub lvl on act 4 chap 6 since it's the easiest alone while I had trouble controlling my other controller to shoot these fishies during the 1st checkpoint and 3-4 rams from them caused my sub to blow up lol.
