If you are quick enough with your finishers then this should come naturally across your playthroughs.

When you activate a finisher sequence by using a witchcraft when an enemy is low on health, the B button will be surrounded by a blue circle. That is the moment to press B to do a perfect finisher. You usually have to wait around half a second for it to actually happen so don't do the finisher straight away. You'll know if you did it right because Heinrich will turn blue for a couple of seconds.

It has also been noted that it is easier to do a perfect finisher on easier difficulties as the time margin is larger to get it right. So bare that in mind.

To perform a Perfect Finisher, you must press the Circle Button prompt perfectly during the Finisher. Near the end of a spell, a ring of blue light will close in on the button. When the glows blue, that's your cue to press it. If done successfully, Heinrich will have a blue aura surrounding him during and after the Finisher move, indicating you performed a Finisher perfectly.

Fortunately, the lower the difficulty you play on, the easier you can perform a Perfect Finisher since the window of opportunity is larger. If you have trouble earning this on Knight or higher, consider cleaning this up on a lower difficulty if you still don't have it by the end of your third playthrough.

Perform a Perfect Finisher 100 times, and the trophy will unlock.

A perfect finisher is when you press the prompt with perfect timing during a Finisher. There is a larger window of opportunity for this on easier difficulties. Heinrich will glow blue if you execute a perfect Finisher.
