How to unlock the The One and Only achievement in Mass Effect 3 - Definitive Guide
This is basically playing you against yourself (your Shepard that is) while the enemy Shepards use different classes.
Now before I post the way I did it I guess that Tomjeh's way (as he pointed out in my "Simulated Hero" guide) will makes this quite easy.
It's basically setting the combat difficulty in the options menu to "Narrative", beat waves 1 & 2 and let one enemy live in wave 3.
Now switch the combat difficulty back to "Normal" and kill the last enemy, achievement should pop now.
I didn't do it myself so I can't confirm whether or not this works.

Now to the way I did it:
- Map: Wingman
- Enemy: Mirror Match
- Score Modifier: Enhanced Damage Boost
When the match starts, turn left and run to the far corner on the lowest level.
Position yourself on the very last cover point and your squad mates on the two cover points before you.
I used Garrus and Liara while I was playing Sentinel.
Liara is very good for biotic combos while Garrus is good to get shields down.
The most difficult part is that the enemies tend to rush your place, so you will need to make good use of powers like Overload or Biotic Explosions to stagger them.
Also, it seemed to me like some powers can hit you even behind cover and stagger you, what makes you vulnerable for more biotic attacks which can basically drag you out of cover.
If you keep your head in cover and watch your left and right to not get flanked, this should go quite well.
Usually, the Infiltrator was the last enemy standing so keep an eye for your cloaked clone.
I would also recommend using the M-7 Lancer as it has infinite ammo, so you won't need to run for ammo boxes.
You're not going for points here therefore don't go for the bonus pickups in between waves as that can make your previous position an enemy spawn zone.
That's about it, might take a few tries but I got it on my first try this way.
Wasn't that hard after all if you keep any eye open not getting flanked.

Shepard: M-7 Lancer V, with AR Piercing Mod IV and AR Magazine Upgrade V; Cerberus Nightmare Armour.
Javik: Particle Rifle V, with AR Stability Damper V; AR Piercing Mod IV.

Strategy: Play smart. I don't think I did anything particularly clever. Stick to cover as best as you can, and go from cover to cover when the grenades are thrown at you. Keep distance between you and the enemies, and use the terrain to your advantage for breaking their line-of-sight. As with the rest of Insanity in ME3, and so too on lower difficulties, make use of your squad powers - Grunt's Concussive Shot is very useful for setting off Fire Explosions, primed by Shepard's Incendiary Ammo, and Javik's Biotic abilities keep the enemies debuffed.
I found the fight at the end of the DLC to be harder, for comparison. Grunt and Javik were particularly useful because they actually engaged the enemy, unlike some squadmates that just seem to stand around.

I imagine you've since found out where it is, but if not, it's part of the Citadel DLC - opposite the entrance to the apartment complex as you walk onto the Silversun Strip. It's Point of Interest 5 on the map.
You can do it the hard way, actually doing it on normal (hardcore or insanity), or drop the combat down to narrative and head into the simulator.
You have to have bought the Mirror Match Combat set from the gold winning terminal.
So choose that under enemies, choose a map, and some squad members and fight.
I chose to use the Wingman map because it's open and easy to maneuver in. I also chose to turn on the Player Damage Boost score modifier to make the match a bit easier.
Squad members chosen were Liara and Garrus. Liara for her biotics and Garrus because I just like Garrus. As I was an Engineer, I threw out a lot of Combat Drones and Sentry Turrets. As for weapons, M-7 Lancer is great because of unlimited ammo.
The simulator Shepards' can be a bit annoying because they like to spam their attacks and run up to you. Try to avoid them and find good cover; their biotics and powers are pretty damaging as they cause staggering and can make you vunerable, even on narrative.
Go through the waves and once you hit the last one or two enemies, bring the combat difficulty up to normal and finish the simulator. Trophy should pop as you load into the arena lobby.
Fortunately, there's a fairly easy way to do this. Play on Casual or Narrative all the way until the very last enemy in the last round. When there is one left, pause and increase the difficulty to Normal, Hardcore, or Insanity. Even with playing on the lowest difficulty, the Spectre-level enemies are still pretty tough, so be prepared for a challenge. When there is one enemy left, there will be a a red diamond on them showing you where they are.
Round 1 - Soldiers, Engineers, and Adepts
Round 2 - Vanguards, Infiltrators, and Sentinels
Round 3 - Adepts, Sentinels, Soldiers and Adepts
To unlock this achievement, you need to earn a gold level token, by scoring quite high in a match. After you have acquired one, you need to spend it on 'Mirror Match Combat Set' from the gold level token store.
When you have done this, you will need to set these as your enemy in a match and defeat them on normal or higher difficulty and the achievement is yours.
*Note, if you are struggling with this achievement, you can play the match on casual/narrative difficulty, then when there is only 1 enemy left, switch the difficulty back to normal and kill the last enemy and you will still get credit for completing it on normal.
In order to unlock Spectre-level opponents, you'll need to earn a gold prize in the Arena. You'll need to get over 6000 points in one match. Once you do, go to the gold rewards booth and unlock Mirror Match. When you go down to the simulator computer, select N7 enemies. Everything else can be set at random or whatever you want to choose. For the squadmates, I decided that three tanks are better than one, so I brought in Wrex and Grunt. You will need them for this challenge. Why is it a challenge? Because you're facing multiple Shepards of different classes at the same time.

- don’t forget that you can’t change the difficulty if you go for platinum for a series on the Madness difficulty (or leave the save for later and go through on normal)
- In madness I mastered the 20th attempt: partners with overloaded shields, armor-piercing/warp cartridges and anti-armor skills (warp can be upgraded for additional damage and weakening of armor)
- It’s better to kill one at a time: remove the shield, use skills against armor and focus damage on your partners and your own
- It’s better to keep your distance and change covers, because the Shepards will shoot you with abilities and throw grenades at you
