How to unlock the Psychopath achievement in Mortal Kombat 11 - Definitive Guide
The fastest way to grind out 100 brutalities is to set Rounds to 1 in settings, then start a local versus with any character on Very Easy. Now beat down your opponent without blocking and make sure to hit an uppercut on the last hit ( + hold ). After hitting the brutality simply hit retry and repeat.

You also have to Hold Y during the uppercut to get the Brutality.

You can also do this with 2 controllers to make it easier.

If you have the Sinking Feeling brutality for Geras than with the proper AI set up, you can have the AI farm brutalities on the survivalist tower for you.
The AI build I use is 30 points in both Zoning and Runaway and then only equip the Quick Sand ability on Geras and nothing else. Then load up a Survivor tower on Very Easy and select this Gerus build and turn on AI Fighter. The AI settings will make it so that the AI fighter will try and constantly move away from the opponent and use ranged attacks, and since we only equipped Quick Sand on him that's the only ranged attack he has. If he finishes the fight with the Quick Sand attack than it will do a brutality (once again, ONLY if you have unlocked the Sinking Feeling brutality already). The only thing you need to do is hit after every fight to progress to the next one.
Side note: the AI won't always do a Brutality, if the AI ends the fight with any other attack it will do a fatality instead, or if the Sinking Feeling final hit is a crushing blow than it might ruin the Brutality. Despite all that however, a vast majority of the time the AI will get a Brutality, in the 25 fights Survival tower my AI ends up averaging around 15-18 brutalities.
This isn't as fast as manually doing very easy towers yourself or using a second controller in local vs mode to grind it out, but I prefer this because I can do other things while the AI grind it for me, as long as I'm there to hit .
BONUS: This also works as a good heart grinding method. A Brutality gives 5 hearts and a fatality gives 3, if you use the tower consumable that doubles your hearts earned, than its 10 and 6 respectively and could farm around 175-225 hearts per tower just having the AI do it for you.

In this video guide I will show you how to get 25 brutalities. 100 brutalities, and 10,000 pints of blood spilled. Load up klassic towers, pick the first tower, pick anyone you want, and put it on very easy. Once the game loads up pause the game and look at the finishing moves page so you know what to do when the time comes.
To perform any easy brutality you will need to not block during the second round, hold the Y button, and press down on the d-pad + the Y button. You will only need to get their health bar down far enough that the uppercut will kill them and just make sure you have the requirements. The brutality should take their head off along with the spine for to to be a brutality. The achievement will pop once the brutality is done.
The pints of blood should follow shortly aster if you keep farming for the brutalities. Should only be several matches after the brutality achievement pops. Achievement will pop once the match ends.

If you would like to run AI I found SubZero can rack a ton of brutalities and from my amount of time I think maybe he does the most. Setting AI Behavior to 30 combo, 30 reversal, does the trick for me.
This isn't as fast as manually doing very easy towers!
Subzero has several brutalities he is fairly likely to hit but for some reason with the AI behavior set this way he goes for a ridiculous amount of uppercuts.
Bonus Tip: Setting AI behavior and then having any run through character towers might be the way to go on this, for completing all five character challenge towers (top left towers of time) you will receive an intro, victory, and loads of customizations.

Brutalities are another type of finisher that first appeared in the console versions of Ultimate Mortal Kombat 3. These finishers can only be triggered when certain conditions are met, such as performing a specific number of throws, doing certain actions for a specified number of times, being under a specific amount of health and even rapidly pressing specific buttons (or holding a specific button down) to execute the brutality.
These finishers cannot be done during the "FINISH HIM!/HER!" sequence at the player's match point but rather, during the match as long as the opponent's health is low. An easy brutality you can perform is The Klassic, which can be done by doing an uppercut attack () while holding down . You can also unlock additional brutalities for each character when certain conditions are met.
As the trophy's description says, you will need to do 100 Brutalities in order to unlock it. You can use the same brutality repeatedly and it will count towards the trophy. In addition, pints of blood spilled that are spilled from brutalities will also count towards the Blood In The Water trophy. You can also keep track of your progress towards this trophy by pressing to open up your Kombat Kard in the main menu, then go to Kareer Stats.

If you have the “Sinking Feeling” brutality for Geras, then you can automatically get this trophy with the right AI customizer in the survival tower.
Create a new striker subclass for Geras, in the AI settings set 30 points on Zoning and Runaway, leaving only the “Quick Sand” attack from special attacks. Launch the survival tower in classic mode, on a very easy difficulty level and turn on the AI fighter. With the previously set settings, a fighter under AI control will constantly move away from the enemy and use only long-range attacks, and we left him only “Quick Sand”, which at the same time coincides in combination with the brutality of “Sinking Feeling”. Thus, he will almost always finish off the enemy with this brutality in the end, the only exceptions will be situations when he manages to finish off the enemy with any other attack, in which case there will be a fatality. All you have to do is click at the end of each battle to start the next one.
Of course, you can do it faster by hand, but this method is also good for collecting hearts. Each brutality gives 5 hearts, fatalities only 3. For one tower out of 25 battles, Geras usually makes from 15 to 20 brutalities, this is from 175-250 hearts per tower, if you use consumables that increase the number of hearts received per tower.

choose any fighter and finish with an uppercut without waiting for Finish Him (the main thing is not to block in the last round)

You can knock out a trophy the old fashioned way. Set the game settings to one round. We connect a second gamepad and gain brutality in local battles. Fast and easy.

It's easy to launch on classic towers, select the "Very Easy" difficulty, finish with an uppercut and you always get Brutality.
I scored 50 myself, I continue)
