Can be done with two contollers. Turn play clock off. Then as main profile score run out the back of your endzone making you being behind 0-2. Then Kickoff and have the other conroller kick it back to you have possesion. Kneel twice ( once in the third quarter and once in the 4th. Make sure you are at about the 10-15 yardline making the field goal 27-32 yards. Wait until 2 seconds are left and call timeout with controller 2. Your kicker is now iced. Make the field goal and POP! Also as stated above you can get the ach for winning the game as time expires as you go ahead 3-2!

Play a game in any mode, but keep the score close. Try to keep your score within three points of your opponent to increase the odds that he will try to ice your kicker.
If you set the game for the shortest possible quarters, it's easy to keep the game scoreless by managing the clock and optionally turning off the penalties so that you can position your defender over the line of scrimmage for easy sacks against the CPU.
Late in the fourth quarter, get within field goal range, being careful not to let the CPU burn all their time outs before you get there. With the game on the line, the CPU will call a time out before you kick, "icing" your kicker. The kick meter will be covered with a block of ice, making it difficult to read. If you are familiar with the timing of the kick meter, or within a very short distance to the goal line, this Field Goal should be easy to make, and the Achievement will unlock.
Play against a team with a weak defense, and adjust your sliders to make the game easier to score if you need the additional help.

You can ice your own kicker and get the achievement as well.

Set the Quarters to 1 Minute, set the Difficulty to Freshman. Run down the clock and waste time until the 4th Quarter. Now once you get to the 4th Quarter, make sure you it is a tied game. Get within the 30 and 20 yd. line and then call a Timeout with 0:02 seconds left. Once you do that, the AI will call a timeout also to try and Ice you, to get you off track, but it won't work. Now once they call that timeout the field goal meter is gonna be iced, but you can still see it to where you can make the field goal, just make the field goal and the achievement will unlock for you.
Also you can get this and the 'Ice In Your Veins' achievement at the same time.
