How to unlock the BOWELWHISTLER achievement in Outlast - Definitive Guide
The first being that insane is much like nightmare difficulty so a few practice runs would be advised for those not up to the challenge. The second being that enemy's are much more aggressive and will react to you much faster and be able to one hit you. The third being that you will only have 2 batteries for your camera and even then, they will be very difficult to find. And the fourth being the worst and that is that if you die by falling or being killed by the inmates.... You have to start it all over from the moment you break out of the chair which by then though if you watch the below video, should have memorized the route in one go.
All Achievements for the DLC can be earned in a single run and can be finished in 32 minutes (from what my camera showed, not including cutscenes).
Below is a video of a speed run which helped me learn the route and get past my sick and twisted husband-to-be due to his annoying one hit kills and my legs issue. But good luck fellow achievement hunters and if you down voted please leave a comment as to why :)
the out of bounds skips are easier to do in whistleblower consistantly skipping every major encounter for insane, so hopefully this helps while this is also my current world rekkie run.
Similar to the base game, there is an insane mode. Insane mode is the hardest difficulty. It's Nightmare difficulty with no checkpoint or saves. This will require you to finish the game without dying and you have to do it in a single sitting. You can pause the game so if you need a break for any length of time, you can do that. But if you die at any point in the game, you will respawn right back in your cell where you get the camcorder in the beginning of the game. If you die from a failed jump, you will have to start over.
Also make sure you manage your battery power. There are far fewer battery pickups but as long as you don't use the night vision unnecessarily, you won't have any issues. Just remember that you only have to turn it on for no more than a second or two at a time to keep you pointing in the right direction.
You can play the game on Nightmare difficulty first to get an idea of what to expect since it's the same as Insane. On Nightmare, you can die and make use of checkpoints so it's a perfect opportunity to practice particular sections. If a section does pose an issue for it, get there on Nightmare difficulty and create a manual save and practice it and reload the save over and over again until you can effortlessly pass the section without dying. Overall, this is not as difficult as the main game was.
As for batteries, at the very beginning when you get let out of your cage and exit the next door, go to the room just to the right and there will be one there. This should be able to get you to the torture scene where you will then have zero battery power when you get your camcorder back. After jumping out the window you will come to the door to the male ward which will require you to find a key. The first door to the right after that will have a battery in it. This should get you to the end of the game.

Overview:
Insane is essentially Nightmare difficulty, but Insane has no checkpoints or saves. You must complete Insane in one sitting, without dying. It may be in your best interest to attempt practice playthroughs on Nightmare before starting Insane, so that way when you get around to Insane you’ll have some strategies for handling certain scenarios. With a little practice you should be able to complete Whistleblower in about 45-50 minutes, so it is a bit shorter than Outlast.
If you end up dying on Insane, the game starts you back in your cell in the Hospital chapter. However you must play through the Underground Lab portion again or else you won’t earn the achievement. So if you die, exit the game then start a new game on Insane.
Generic Gameplay Tactics:
There are at least five distinct enemies in Whistleblower.
The Walrider will kill you in two hits soon after you exit your cell at the start of the game if you don’t get away quick enough. He hits you once and this first blow is unavoidable, so quickly make a left down the hall and open the first door on the left side of the hallway before he can hit you again. The Cannibal (variant with the handheld electric saw in the Hospital chapter) will also kill you in two hits.
Other than those two major enemies, Chris Walker (big guy from the main game), the Naked Twins, and Eddie Gluskin (The Groom) will all kill you in one hit. Falling from a great height will also kill you instantly. There are two major jumps in Whistleblower but any jump in the game shouldn't be much of an issue as long as you sprint for a couple of seconds prior to jumping those gaps.
All normal variants will also kill you in two hits.
You only encounter The Twins once. They stay put for the most part and don't chase you down. Just keep your distance from them.
The first time you encounter Chris Walker in Whistleblower will be right after Jeremy Blaire destroys the radio. Walker breaks down the door by throwing a body at it. Immediately walk, don’t run towards the row of four swivel chairs and crouch between the two in the middle. Walker will come into the room. Wait for him to walk further in, then get up and run the way he came. Practice this on Nightmare if you must. This scenario takes place right at the start of the Prison chapter.
The second and final time Walker appears in Whistleblower is a fairly straightforward situation, just keep running down the hallway, jump over the obstacles, then hop out the window and you should be fine.
As for The Cannibal and The Groom, there aren't any “ideal” strategies you can use to get by them. The Groom is without a doubt trickier to deal with as he can kill you in one hit. When you have to progress forward and they're blocking the way, try to get their attention and have them follow you. Lead them to a table or something and run around it, then come back from the way where they were blocking the path forward. In the Hospital chapter with The Cannibal, there are a few beds around so use those to hide if you must. When The Groom is chasing you in the Vocational Block always shut doors behind you in order to give yourself a few extra seconds to run away.
Battery locations:
Thankfully, there’s no ENERGISER achievement for this DLC, so feel free to use the camcorder’s night vision as often as you want. I made a list of where some batteries can be found for Nightmare/Insane mode (these locations may be different on Normal/Hard). The first three battery pick ups here should be more than enough to get to the torture scene near the end of the game.
1. After grabbing the camcorder for the first time you’ll see that you’re locked in a cell. A variant will come and open the cell door for you. Exit the cell, open the next door, and you’ll be in a hallway. Go through the first door on your right and you’ll be in a restroom. On a cart here is a battery.
2. Eventually you’ll press a button to open up a decontamination chamber. Here, a variant will smash someone’s head against a glass panel (“Doctors and liars!”). Keep moving on and a person will run away from you (“Another one’s coming!”). Just ahead you’ll find a bunch of large silver/gray canisters with the label “Morin Solutions.” Behind these canisters is a battery, grab it before climbing up.
3. As soon as you see Father Martin writing on the wall with blood (Down the drain), hop down the hole and continue. In the room where a guard closes a gate on you (“Oh God, one of them’s coming! It’s not even human anymore.”) there’s another smaller room you can enter. Open the door (it’s got a recycling poster on the wall next to it). Towards the right of this room in the back will be the battery, on a desk.
4. Once the torture scene with The Groom ends, you'll automatically grab the camcorder. Unfortunately, the batteries have been removed! Continue to run away from The Groom and after some time you’ll have to jump out of a window. Soon you’ll come to a locked door that leads to the male ward (objective will then change to finding a key). Move on and immediately go through the first door on your right. There will be a battery on a shelf in front of you.
